肺炎支原体肺炎患儿支气管肺泡灌洗液及血清中IL-6、IL-4、IFN-γ的水平变化及相关性分析

摘　　要：目的:探讨肺炎支原体肺炎(MPP)患儿支气管肺泡灌洗液(BALF)及血清中白介素-6(IL-6)、白介素-4(IL-4)、干扰素-γ(IFN-γ)、IFN-γ/IL-4水平的变化及相关性。方法:选取MPP患儿40例为MPP组,病原体检测阴性患者20例为对照组,比较2组患儿BALF及血清中IL-6、IL-4、IFN-γ、IFN-γ/IL-4水平,探讨其与MPP血液生化指标及住院时长的相关性。结果:MPP组BALF及血清中IL-6、IL-4、IFN-γ水平明显高于对照组;MPP组BALF中IL-6、IL-4、IFN-γ水平明显高于其血清水平,差异有统计学意义(P<0.05);采用受试者工作特征曲线(ROC)分析显示,BALF中IFN-γ评估儿童支原体肺炎的曲线下面积(AUC)为0.950,敏感度95%,特异度57.5%;BALF中IFN-γ联合IL-4、IFN-γ/IL-4的AUC为0.969,敏感度95%,特异度95%。结论:MPP感染时BALF和血清中IL-6、IFN-γ、IL-4存在异常表达,血清和BALF中各细胞因子水平呈正相关,MPP患儿BALF中IFN-γ、IL-4、IFN-γ/IL-4可作为预测儿童MPP病情的指标。Objective:To investigate the changes and correlation of IL-6,IL-4,IFN-γ,and IFN-γ/IL-4 levels in bron-choalveolar lavage fluid(BALF)and serum in children with Mycoplasma pneumoniae pneumonia(MPP).Methods:A total of 40 children with MPP were selected as the MPP group,and 20 cases of negative pathogen test were selected as the control group.The levels of IL-6,IL-4,IFN-γ,and IFN-γ/IL-4 in BALF and serum were compared between the two groups.The correlation be-tween the blood biochemical indexes of MPP and the length of hospitalization was discussed.Results:The levels of IL-6,IL-4,and IFN-γin BALF and serum of the MPP group were significantly higher than those in the control group.The levels of IL-6,IL-4,and IFN-γin BALF in the MPP group were significantly higher than those in serum(P<0.05).The receiver operator char-acteristic curve(ROC)analysis showed that:the area under the curve(AUC)of IFN-γin BALF assessed mycoplasma pneumonia in children was 0.9500,with a sensitivity of 95%and specificity of 57.5%.The AUC of IFN-γcombined with IL-4 and IFN-γ/IL-4 in BALF was 0.9688,and the sensitivity and specificity were both 95%.Conclusion:When MPP infection occurs,IL-6,IFN-γ,and IL-4 in BALF and serum are abnormally expressed,and the levels of cytokines in serum and BALF are positively cor-related,and IL-6,IL-4,IFN-γ,and IFN-γ/IL-4 in BALF can be used in predicting MPP in children.
