Q开关Nd:YAG激光双波长交替治疗太田痣疗效分析

摘　　要：目的:探讨Q开关Nd∶YAG激光1064 nm联合532 nm双波长交替治疗太田痣的效果。方法:回顾性分析2018年2月-2021年2月笔者医院收治的591例太田痣患者的临床资料,依据不同激光治疗方案分为长波组(n=287,Q开关Nd∶YAG 1064 nm激光治疗)和交替组(n=304,Q开关Nd∶YAG激光1064 nm联合532 nm双波长交替治疗)。比较两组临床疗效,治疗次数,总治疗时间和不良反应发生情况,使用温哥华瘢痕量表(Vancouver scar scale,VSS)比较两组皮损情况,使用一般自我效能量表(General self-efficacy scale,GSES)和自尊量表(Self-esteem scale,SES)比较两组自我效能和自尊水平。结果:交替组和长波组患者美容总有效率分别为97.70%和96.17%(P>0.05)。两组总治疗次数差异无统计学意义(P>0.05),交替组总治疗时间短于长波组(P<0.05);治疗后两组VSS评分均较治疗前降低,且治疗后交替组不同时间点VSS评分均低于长波组,存在时间、组间和交互效应(P<0.05);交替组不良反应的发生率为4.28%,低于长波组的8.36%(P<0.05);治疗后,两组GSES和SES评分均高于治疗前(P<0.05),但两组差异无统计学意义(P>0.05)。结论:Q开关Nd∶YAG 1064 nm联合532 nm双波长激光交替治疗太田痣效果明显,长短波交替治疗能够缩短治疗时间,有效促进瘢痕恢复,且有助于患者提高其自我效能和自尊水平。Objective To analyze the effect of Q-switched Nd∶YAG laser of 1064 nm wavelength combined with 532 nm wavelength in the treatment of nevus of Ota.Methods The clinical data of 591 patients with nevus of Ota admitted to the hospital were retrospective analyses between February 2018 and February 2021,and the patients were divided into long wave group(Q-switched Nd∶YAG laser of 1064 nm wavelength,n=287)and alternating group(alternating treatment with Q-switched Nd∶YAG laser of 1064 nm wavelength combined with 532 nm wavelength,n=304)according to different laser treatment regimens.Comparing the clinical efficacy,number of treatments,total treatment time and incidence of adverse effects between the two groups.The vancouver scar scale(VSS)was used to compare the scar status in the two groups.The complications were compared,and General Self-Efficacy Scale(GSES)and Self-Esteem Scale(SES)were adopted to compare the self-efficacy and self-esteem.Results The total cosmetic effective rate was 97.70%in alternating group and that in long wave group was 96.17%(P>0.05).There was no statistical significance in the total number of treatments between the two groups(P>0.05),and the total treatment time in alternating group was shorter than that in long wave group(P<0.05).After treatment,the VSS scores of the two groups were reduced compared with those before treatment,and the VSS scores in alternating group at different time points after treatment were lower than those in long wave group,with time-point effect,between-group effect and interaction effect(P<0.05).The incidence rate of adverse reactions was 4.28%in alternating group,which was lower than 8.36%in long wave group(P<0.05).After treatment,the scores of GSES and SES in the two groups were higher than those before treatment(P<0.05),but there were no statistically significant differences between the two groups(P>0.05).Conclusion Q-switched Nd∶YAG laser of 1064 nm wavelength alone and alternating treatment of combined with 532 nm wavelength both have obvious effects in t
