良愿前置视角下同理心护理对乳腺癌保乳整形患者负性情绪及社会适应性的影响

摘　　要：目的:探讨良愿前置视角下同理心护理对乳腺癌保乳整形患者的影响。方法:选择2019年1月-2022年1月于笔者医院行保乳整形治疗的110例乳腺癌患者为研究对象,采用随机数表法分为观察组(n=54)及对照组(n=56)。对照组给予常规护理,观察组在对照组基础上给予良愿前置视角下同理心护理,比较两组患者乳房美学效果、负性情绪[汉密尔顿焦虑量表(Hamilton's anxiety,HAMA)、汉密尔顿抑郁量表(Hamilton depression,HAMD)]、自尊状态[自尊量表(Self esteem scale,SES)]、社会适应性(心理社会适应性能力测评问卷)、生活质量[乳腺癌患者生命质量测定量表(Functional assessment of cancer therapy-breast cancer,FACT-B)]及癌因性疲乏状态(Piper疲乏量表)评分。结果:护理干预后,两组患者乳房美学效果优良率比较差异无统计学意义(P>0.05);观察组HAMA、HAMD及癌因性疲乏状态评分低于对照组(P<0.05),SES、FACT-B及社会适应性评分高于对照组(P<0.05)。结论:将良愿前置视角下同理心护理应用于乳腺癌保乳整形患者护理中,可显著改善患者情绪状态、生活质量、癌因性疲乏状态,提高自尊感及社会适应性,值得在临床推广应用。Objective The aim of this study was to investigate the impact of empathy nursing from the perspective of leading by the good will of patients undergoing breast-conserving surgery for breast cancer.Methods A total of 110 patients with breast cancer who underwent breast-conserving plastic treatment in the author's hospital from January 2019 to January 2022 were selected as the research objects.They were divided into the observation group(n=54)and the control group(n=56)by the random number table method.The control group was given routine nursing,and the observation group was given empathic nursing under the perspective of leading by the good will on the basis of routine nursing.The breast aesthetic effect,negative emotions[hamilton's anxiety scale(HAMA),hamilton depression scale(HAMD)],state of self-esteem[self-esteem scale(SES)],social adaptability(psychosocial adaptability assessment questionnaire),quality of life[functional assessment of cancer therapy-breast cancer(FACT-B)]and cancer-related fatigue(Piper fatigue scale)were compared between the two groups.Results After nursing intervention,there was no significant difference in the excellent and good rate of breast aesthetic effect between the two groups(P>0.05).The HAMA,HAMD and cancer-related fatigue scores of the observation group were lower than those of the control group(P<0.05),and the SES,FACT-B and social adaptability scores were higher than those of the control group(P<0.05).Conclusion The application of empathy nursing from the perspective of leading by the good will in nursing care of breast cancer plastic surgery patients can significantly improve the emotional state,quality of life,cancer-related fatigue,self-esteem and social adaptability of patients,and is worthy of clinical application.
