摘 要:目的:探讨低管电压、管电流、对比剂剂量模式的腹部CT血管造影检查(CTA)的可行性。方法:选取2021年1月至2022年1月在我院行腹部CTA检查的患者60例,采用随机数字表法分为观察组(n=30)与对照组(n=30),观察组以低管电压100kV、低管电流200mAs、低对比剂剂量与浓度50mL(320mgI/mL)模式行腹部CTA检查,对照组以管电压120kV、管电流250mAs、对比剂剂量与浓度90mL(320mgI/mL)模式行腹部CTA检查。比较两组腹部各血管部位(L2水平腹主动脉、腹腔干、右肾动脉、肠系膜上动脉、右髂内动脉)CT值,客观质量评价指标(腹主动脉对比信噪比与信噪比、背景噪声),辐射剂量相关指标[容积CT剂量指(CTDIvol)、剂量长度乘积(DLP)、有效剂量(ED)、总碘量]及以腹主动脉CTA评分比较M(Q1~Q3)。结果:观察组腹部各血管部位CT值均高于对照组(P<0.05);两组观察组客观质量评价指标比较差异无统计学意义(P>0.05),观察组辐射剂量相关指标均低于对照组(P<0.05);Kappa一致性检验显示,两名专业医师对两组腹部CTA图像评分一致性优良,两组腹主动脉CTA评分比较差异无统计学意义(P>0.05)。结论:低管电压、管电流、对比剂剂量模式的腹部CTA可在一定条件下满足常规诊断要求,有效下调了辐射剂量,减轻了对患者辐射伤害,具有良好推广应用价值。Objective:To investigate the feasibility of abdominal CT angiography(CTA)with low tube voltage,tube current and contrast agent dose mode.Methods:60 patients who underwent abdominal CTA examination in our hospital from January 2021 to January 2022 were selected and divided into observation group(n=30)and control group(n=30)by random number table method.Abdominal CTA was performed in the mode of tube voltage 100 kV,low tube current 200 mAs,and low contrast agent dose and concentration 50 mL(320mgl/mL).Abdominal CTA was performed in mL(320mgl/mL)mode.The CT values of various abdominal vascular sites(L2 level abdominal aorta,celiac trunk,right renal artery,superior mesenteric artery,and right internal iliac artery)were compared between the two groups.Background noise),radiation dose-related indicators[volume CT dose index(CTDI_(vol)),dose-length product(DLP),effective dose(ED),total iodine]and abdominal aortic CTA score comparison M(QI~Q3).Results:The CT values of each abdominal blood vessel in the observation group were higher than those in the control group(P<0.05);there was no significant difference in the objective quality evaluation indexes between the two groups(P>0.05),and the radiation dose-related indexes in the observation group were lower than those in the control group.group(P<0.05);Kappa consistency test showed that the two professional physicians had excellent consistency in the scores of abdominal CTA images between the two groups,and there was no significant dference in the abdominal aorta CTA scores between the two groups(P>0.05).Conclusion:Abdominal CTA with low tube voltage,tube current and contrast agent dose mode can meet the requirements of routine diagnosis under certain conditions,effectively reduce the radiation dose,and reduce radiation damage to patients,which has good application value.
