痔上黏膜环切术联合外痔切剥术治疗Ⅲ-Ⅳ度环状混合痔患者的疗效观察

摘　　要：目的分析Ⅲ-Ⅳ度环状混合痔应用痔上黏膜环切术(PPH)联合外痔切剥术治疗的效果。方法按随机数字表法将2020年1月-2022年12月湖北省第三人民医院胃肠外科收治的66例环状混合痔患者分为对照组(外痔切剥术治疗,33例)和研究组(PPH联合外痔切剥术治疗,33例),对比两组围术期指标、疼痛程度、并发症情况。结果研究组术后疼痛、创面愈合及住院时间较对照组更短,术中出血量较对照组更少(P<0.05);研究组术后1d、3d、5d及7d视觉模拟评分法(VAS)评分较对照组更低(P<0.05);研究组并发症发生率6.06%较对照组的30.30%更低(P<0.05)。结论Ⅲ-Ⅳ度环状混合痔应用PPH联合外痔切剥术治疗,术后疼痛程度轻,并发症少,有利于患者及早恢复。Objective To analyze the efficacy of procedure for prolapse and hemorrhoids(PPH)combine with external hemorrhoidectomy in the treatment of grade Ⅲ to Ⅳ mixed hemorrhoids.Methods 66 patients with circular mixed hemorrhoids who were admitted to the Gastrointestinal Department of the Third People's Hospital of Hubei Province from January 2020 to December 2022 were divided into control group(treated with external hemorrhoidectomy,33 cases)and study group(treated with PPH combined with external hemorrhoidectomy,33 cases)by random number table method,the perioperative indicators,pain degree,and complications were compared between two groups.Results Study group had shorter time of pain,wound healing,and hospitalization after surgery compared with control group,less intraoperative bleeding volume compared with control group(P<0.05).The visual analogue scale(VAS)scores in study group were lower than those in control group 1d after surgery,3d after surgery,5d after surgery,and 7d after surgery(P<0.05).The incidence of complications in the study group was 6.06%lower than that 30.30%in the control group(P<0.05).Conclusion The application of PPH combine with external hemorrhoidectomy in the treatment of grade Ⅲ to Ⅳ circular mixed hemorrhoids,the pain degree after surgery is mild,less complications,and which is beneficial for patients to recover early.
