磁共振REACT技术在下肢大隐静脉曲张诊断中的价值

摘　　要：目的:探讨磁共振无对比剂和心电、呼吸门控触发的血管造影(REACT)技术在下肢大隐静脉曲张诊断中的价值。方法:选择2020年9月至2021年10月于本院收治的22例(共31段静脉)下肢大隐静脉曲张患者作为研究对象,均完成磁共振REACT、彩超和下肢静脉造影检查,以下肢静脉造影诊断结果为金标准,分析不同检查方法对下肢大隐静脉曲张的诊断价值。结果:经诊断,下肢静脉造影诊断出局部深静脉栓塞1例,深静脉血栓后遗症2例,单纯深静脉瓣膜功能不全7例,深静脉正常12例,磁共振REACT及彩超对下肢大隐静脉曲张的诊断结果比较,无明显差异(P>0.05);磁共振REACT及彩超均诊断出下肢静脉曲张为右侧曲张4例,左侧曲张2例,双侧曲张16例;磁共振REACT诊断下肢大隐静脉曲张的图像质量信噪比(SNR)和对比信噪比(CNR)分别为(346.14±47.46)和(159.23±36.39),其诊断敏感度、准确率均较高,且磁共振REACT诊断下肢大隐静脉与下肢静脉造影诊断的kappa值为0.761,一致性较高。结论:磁共振REACT对下肢大隐静脉曲张的诊断能直观显示两侧髂静脉及以下静脉分支全貌,获得静脉形态结构变化的高质量图像,相较于彩超具有较高的诊断效能。Objective:To investigate the value of magnetic resonance angiography without contrast agent,electrocardiogram and respiratory gated angiography(REACT)in the diagnosis of lower extremity saphenous varices.Methods:Twenty-two patients with varicose veins of lower extremity(31 segments of veins)who were admitted to our hospital from September 2020 to October 2021 were selected as the research subjects.The diagnostic results of angiography were the gold standard,and the diagnostic value of different examination methods for varicose veins of the lower extremity was analyzed.Results:After diagnosis,1 case of local deep vein thrombosis,2 cases of deep vein thrombosis sequelae,7 cases of pure deep vein valve insufficiency,and 12 cases of normal deep vein were diagnosed by lower extremity venography.There was no significant difference in the diagnosis results(P>0.05).Magnetic resonance REACT and color Doppler ultrasound both diagnosed 4 cases of right varicose veins,2 left varicose veins,and 16 bilateral varicose veins.Magnetic resonance REACT diagnosed lower limb varicose veins in 16 cases.The image quality signal-to-noise ratio(SNR)and contrast signalto-noise ratio(CNR)of varicose veins were(346.14±47.46)and(159.23±36.39),respectively,with high diagnostic sensitivity and accuracy.Magnetic resonance REACT was used to diagnose lower extremity.The kappa value of saphenous vein and lower extremity venography was O.761,which was consistent.Conclusion:Magnetic resonance REACT for the diagnosis of saphenous varicose veins of lower extremities can directly display the whole picture of the iliac veins on both sides and the vein branches below,and obtain high-quality images of the changes of vein morphology and structure.Compared with color Doppler ultrasound,it has a higher diagnostic efficiency.
