摘 要:目的:探讨彩色多普勒超声检查特发性真性性早熟(ICPP)女童子宫及卵巢形态与性激素分泌水平的相关性。方法:选取2019年6月至2021年6月我院超声影像科收治的经促性腺激素释放激素(GnRH)激发试验确诊ICPP女童患者共64例。所有患者均行彩色多普勒超声检查及性激素测定,分析超声检查结果(子宫长径、容积、内膜厚度、卵巢长径、容积、最大卵泡直径)与性激素水平(黄体生成素:LH;促卵泡激素:FSH)相关性。结果:与正常女童相比,ICPP患儿的的子宫/卵巢长径、横径、容积和FSH、LH的激素水平显著升高(P<0.05)。患儿子宫长径、横径、卵巢长径及横径与FSH、LH无相关性(P>0.05)。超声检查子宫容积(FSH:r=0.572,P=0.000;LH:r=0.618,P=0.021)、内膜厚度(FSH:r=0.456,P=0.014;LH:r=0.542,P=0.000)、卵巢容积(FSH:r=0.512,P=0.011;LH:r=0.622,P=0.000)、最大卵泡直径(FSH:r=0.572,P=0.000;LH:r=0.572,P=0.012)与FSH、LH水平具有正相关性(P<0.05)。结论:ICPP患儿超声检查子宫及卵巢容积及子宫内膜厚度与性激素具有相关性,在ICPP患儿诊断中具有一定的价值,对于无法完善性激素检测的地区及科室进行ICPP早期诊断有重要价值,可为患儿病情提供重要的临床信息。Purpose:To explore the relationship between uterine and ovarian ultrasonographic morphology and sex hormones in idiopathic central precocious puberty(ICPP)girls.Methods:A total of 64 female patients with ICPP confirmed by gonadotropin-releasing hormone(GnRH)stimulation test admitted to our department from June 2019 to June 2021 were analyzed.All patients underwent color Doppler ultrasonography and sex hormones measurement,and the ultrasound results(the long and short diameter of uterine,the uterine volume,the thickness of endometrial,the long and short diameter of ovarian,the ovarian volume,and the diameter of the largest follicle)were collected,and the correlation between ultrasound results with sex hormone levels(follicle stimulating hormone,FSH;luteinizing hormone,LH)were analyzed.Result:Compared with normal girls,the uterine/ovarian long diameter,transverse diameter,volume and hormone levels of FSH and LH in ICPP children were significantly increased(P<0.05).There was no correlation between the short and long diameter of uterus and ovary and sex hormone levels(P>0.05).Ultrasonographic examination of uterine volume(FSH:r=0.572,P=0.000;LH:r=0.618,P=0.021),the thickness of endometrial(FSH:r-0.456,P=0.014;LH:r-0.542,P=0.000),the ovarian volume(FSH:r-0.512,P=0.011;LH:r=0.622,P=0.000),and the diameter of the largest follicle(FSH:r=0.572,P=0.000;LH:r-0.572,P=0.012)was positively correlated with FSH and LH levels(P<0.05).Conclusion:Color doppler ultrasound examination of uterine and ovarian volume and the thickness of endometrial in female children with ICPP is correlated with sex hormones,which has certain value in the diagnosis of children with ICPP and can provide important clinical information for the disease of children,especially for the regions which the sex hormones examination cannot be performed.
