异丙托溴铵气雾剂联合布地奈德福莫特罗吸入剂治疗慢性阻塞性肺疾病患者的临床疗效

摘　　要：目的 探究异丙托溴铵气雾剂(ipratropium bromide aerosol,IPA)联合布地奈德福莫特罗吸入剂治疗慢性阻塞性肺病(chronic obstructive pulmonary disease,COPD)对患者血气指标、肺功能及运动耐量的影响。方法 选择2020年12月至2022年12月福建省泉州市惠安县张坂镇卫生院收治的62例COPD患者,采用简单随机分组将其分为联合组(IPA联合布地奈德福莫特罗吸入剂治疗,31例)和对照组(IPA治疗,31例),均持续治疗2周。分别于治疗前和治疗3 d后,检测血气相关指标和血清C反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子α(tumour necrosis factor-α,TNF-α)、白细胞介素6(interleukin 6,IL-6)水平。分别于治疗前和治疗2周后,检测肺功能、6 min步行距离(6 minutes walking distance,6MWD)和改良呼吸困难量表(modified British medical research council,mMRC)评分。记录两组不良反应发生情况。结果 联合组和对照组患者治疗后的第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC、动脉血氧分压(PaO_(2))、血氧饱和度(SaO_(2))和6MWD均高于治疗前(P <0.05),动脉血二氧化碳分压(PaCO_(2))和mMRC低于治疗前(P <0.05),血清CRP、IL-6和TNF-α水平低于治疗前(P <0.05);联合组患者治疗后的FEV_(1)、FVC、FEV_(1)/FVC、PaO_(2)、SaO_(2)和6MWD均高于对照组(P <0.05),PaCO_(2)和mMRC低于对照组(P <0.05),血清CRP、IL-6和TNF-α水平低于对照组(P <0.05)。结论 IPA联合布地奈德福莫特罗吸入剂治疗可以促进COPD患者血气指标恢复,提高运动耐量,改善肺功能。Objective To investigate the effects of ipratropium bromide aerosol(IPA) combined with Xinbicobao on blood gas indexes,pulmonary function,and exercise tolerance in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 62 patients with COPD admitted to Fujian Province Quanzhou City Hui'an County Zhang Ban Town Health Center from December 2020 to December 2022 were randomly divided into a combination group(IPA combined with Xinbico Dubao treatment,31 cases) and a control group(IPA treatment,31 cases),all of whom were treated for 2 weeks.Blood gas related indicators,serum C-reactive protein(CRP),and tumor necrosis factor were detected before and 3 days after treatment,respectively α(TNF-α),Interleukin 6(IL-6) levels.Lung function,6-minute walking distance(6MWD),and mMRC scores were measured before and 2 weeks after treatment,respectively.Adverse reactions were recorded in both groups.Results In patients of the combined group and control group,the post-treatment exertional expiratory volume in the first second(FEV_1),exertional lung capacity(FVC),FEV_1/FVC,arterial partial pressure of oxygen(PaO_2),blood oxygen saturation(SaO_2) and 6MWD were higher than before treatment(P<0.05),the blood partial pressure of carbon dioxide(PaCO_2) and mMRC were lower than before treatment(P<0.05),and the serum CRP,IL-6 and TNF-α were lower than before treatment(P<0.05);FEV_1,FVC,FEV_1/FVC,PaO_2,SaO_(2) and 6MWD were higher than those of the control group after treatment in the combination group(P<0.05),PaCO_(2) and mMRC were lower than those of the control group(P<0.05),and serum CRP,IL-6 and TNF-αwere lower than those of the control group(P<0.05).Conclusions IPA combined with Xinbico Dubao treatment can promote blood gas recovery,improve exercise tolerance,and improve lung function in patients with COPD.
