摘　　要：颈椎后纵韧带骨化症是后纵韧带发生病理性异位骨化,缓慢进展的骨化物压迫脊髓、神经根,继而出现一系列神经功能损害的症候群。临床中通过症状体征和影像学检查可明确诊断,其治疗方式的选择依赖于症状的严重性。对于无症状或症状较轻的患者,多采取保守治疗并结合影像学随访;对于出现脊髓或神经病变的患者,则必须采取手术减压治疗。该病的手术入路方式目前主要有前路、后路和前后联合入路3种,不同的手术入路和方式各有其优缺点。颈椎前路椎体骨化物复合体可控前移融合技术是一种治疗颈椎后纵韧带骨化症的新兴术式,经过近几年的推广应用,取得了良好的临床效果。该文结合国内外的临床研究进展,对颈椎前路椎体骨化物复合体可控前移融合技术治疗颈椎后纵韧带骨化症的现状及进展进行综述,为临床工作提供参考。Cervical ossification of the posterior longitudinal ligament(C-OPLL)is a status when pathological heterotopic ossification of the posterior longitudinal ligament occurs,and the slow progressing ossification compresses the spinal cord and nerve roots,followed by a series of neurological impairment syndromes.In clinical practice,a clear diagnosis can be made through symptoms,signs and imaging examinations,and the choice of treatment depends on the severity of the symptoms.For patients with no symptom or with mild symptoms,conservative treatment and imaging follow-up should be taken.For patients with spinal cord or nerve root disease,surgical decompression treatment must be taken.At present,there are three major surgical approaches:anterior approach,posterior approach and combined anterior and posterior approach.Different surgical approaches and techniques have their advantages and disadvantages.The anterior controllable antedisplacement and fusion(ACAF)is a new technique for the treatment of C-OPLL.Through the promotion and application in recent years,it has achieved satisfying clinical results.Based on the clinical studies at home and abroad,this article briefly introduces the current situation and progress of ACAF in the treatment of C-OPLL,providing reference for clinicians.
