摘　　要：目的 分析厦门地区女性人乳头瘤病毒(human papilloma virus, HPV)感染的年龄、亚型分布及多重感染现状,为预防HPV感染和降低宫颈癌的发生率提供理论依据。方法 采用引物多重PCR扩增,流式荧光杂交分型检测的方法对18573名女性患者的宫颈脱落细胞进行HPV基因检测,并分析受检者HPV亚型分布特点,HPV感染的单一、多重感染分布情况以及不同年龄组间HPV感染情况。结果 18573例患者中高危型HPV阳性4926例,总检出率为26.52%,其中单一感染率占55.87%;多重感染率占44.13%。高危型HPV感染率排在前5位的分别为HPV52、HPV53、HPV58、HPV39、HPV16,低危型HPV感染率排在前五位的分别为HPV61、HPV44、HPV81、HPV43、HPV55;单一感染中以HPV52型为主。18573例女性受检者,年龄最小为17岁,最大为87岁,不同年龄段女性HPV感染率比较:≤20岁组感染率最高,>40~50岁组感染率最低,多重感染以≤20岁及>20~30岁年龄组为主;不同年龄组HPV感染亚型分布中,以HPV52型阳性率最高。结论 厦门地区女性HPV感染以单一高危型为主,HPV52、16型多见,不同年龄段的HPV亚型分布不同,主要以HPV52型阳性感染为主,应加强本地区HPV相关健康知识宣传,特别是≤20岁群体的生殖健康教育,并积极科普HPV疫苗的接种,完善宫颈癌的基层预防。Objective To analyze the age,subtype distribution and multiple infection status of human papilloma virus(HPV)infection in women in Xiamen,and to provide theoretical basis for preventing HPV infection and reducing the incidence of cervical cancer.Methods The HPV gene was detected in the cervical exfoliated cells of 18573 female patients by PCR amplification and flow cytometry hybridization,and the distribution characteristics of HPV subtypes,single and multiple infections and HPV infection among different age groups were analyzed.Results There were 4926 high-risk HPV positive cases in 18573 patients,the total detection rate was 26.52%,and the single infection rate was 55.87%;The multiple infection rate was 44.13%.HPV52,HPV53,HPV58,HPV39 and HPV16 were the top five high-risk HPV infection rates,and HPV61,HPV44,HPV81,HPV43 and HPV55 were the top five low-risk HPV infection rates;HPV52 is the main type of single infection.18573 female subjects,the minimum age is 17 years old and the maximum age is 87 years old.Comparison of HPV infection rates among women of different age groups:the infection rate of≤20 years old group is the highest,the infection rate of>40-50 years old group is the lowest,and multiple infections are mainly in the age groups of≤20 years old and>20-30 years old;In the distribution of HPV infection subtypes in different age groups,HPV52 type has the highest positive rate.Conclusion HPV infection in women in Xiamen is mainly single high-risk type,HPV52 and 16 types are common,and the distribution of HPV subtypes in different age groups is different,mainly HPV52 positive infection.We should strengthen the promotion of HPV-related health knowledge in this area,especially the reproductive health education of the group≤20 years old,and actively promote the vaccination of HPV vaccine to improve the primary prevention of cervical cancer.
