术后应用低剂量克拉霉素治疗慢性鼻窦炎对其鼻黏膜、血清ECP及TIgE水平的影响

摘　　要：目的探讨术后应用低剂量克拉霉素治疗慢性鼻窦炎对其鼻黏膜、血清嗜酸细胞阳离子蛋白(ECP)及总免疫球蛋白E(TIgE)水平的影响。方法选取我科2019年9月至2020年6月期间95例慢性鼻窦炎术后患者作为研究对象,按照治疗方法不同分成两组,对照组47例术后给予桉柠蒎肠溶胶囊,观察组48例术后联合低剂量克拉霉素治疗,3个月末观察两组患者鼻黏膜恢复情况、炎症介质、血清ECP及TIgE水平。结果两组治疗后Lund-Kennedy评分、SNOT-20评分分值均降低,但观察组降低幅度高于对照组(P<0.05);两组治疗后血清中IL-6、IL-17、EOS%、ECP、TIgE水平均降低,但观察组降低幅度高于对照组(P<0.05)。结论术后应用低剂量克拉霉素治疗慢性鼻窦炎能抑制炎症反应,促进鼻黏膜恢复,降低血清ECP及TIgE水平。Objective To investigate the effects of low dose clarithromycin on the levels of nasal mucosa,eosinophilic cationic protein(ECP)and total immunoglobulin E(TIGE)in patients with chronic sinusitis.Methods A total of 95 patients with chronic sinusitis after surgery in our department from September 2019 to June 2020 were selected as the research objects and divided into two groups according to different treatment methods.47 patients in the control group were given postoperative capsule containing eucalyptus pinicum,while 48 patients in the observation group were treated with low-dose clarithromycin.At the end of 3 months,nasal mucosa recovery,inflammatory mediators,serum ECP and TICE levels were observed in 2 groups.Results After treatment,Lund-Kennedy score and SNOT-20 score were decreased in both groups,but the reduction rate in observation group was higher than that in control group(P<0.05).After treatment,serum levels of IL-6,IL-17,EOS%,ECP and TIGE in both groups were decreased,but the decrease rate in observation group was higher than that in control group(P<0.05).Conclusion Low dose clarithromycin in the treatment of chronic sinusitis can inhibit inflammation,promote nasal mucosa recovery,and reduce serum ECP and TICE levels.
