脑卒中患者采用自制加长型枕联和良肢摆放在预防压疮和肩手综合症的临床疗效观察

摘　　要：目的分析脑卒中患者应用自制加长型枕联合良肢摆放,在预防肩手综合征以及压疮中的效果。方法选取我院2018年1月至2019年7月收治的78例脑卒中患者为研究对象,随机将患者分为两组常规组和干预组各39例,常规组接受普通枕头以及常规护理,干预组接受自制加长型枕和良肢摆放。将两组干预后肩手综合征、压疮发生率、日常生活能力(ADL)、疼痛、水肿、关节活动度评分进行对比。结果干预后,干预组的压疮为(2.56%)、肩手综合征发生率为(5.13%)低于常规组的(20.51%)(P<0.05)差异有统计学意义;干预后,干预组的ADL评分为(80.55±3.57)高于常规组(70.26±3.10)(P<0.05)差异有统计学意义;干预后,干预组的水肿、疼痛、关节活动度评分为,(0.69±0.09)(1.02±0.47)(2.10±0.57)低于常规组(1.19±0.04)(2.66±0.62)(3.55±1.02)(P<0.05)差异有统计学意义。结论自制加长型枕联合良肢位摆放,明显减少了在脑卒后患者发生压疮以及肩手综合征的情况,相继提高了患者的活动能力,促进了身体恢复。Objective To analyze the effect of self-made lengthened pillow combined with good limb placement in the prevention of shoulder hand syndrome and pressure ulcer in stroke patients.Methods 78 stroke patients admitted to our hospital from January 2018 to July 2019 were selected as the research objects.The patients were randomly divided into two groups:the routine group and the intervention group,with 39 cases in each group.The routine group received ordinary pil-lows and routine nursing,and the intervention group received self-made lengthened pillows and good limbs.The shoulder hand syndrome,the incidence of pressure ulcer,the ability of daily living(ADL),pain,edema and joint activity scores were compared between the two groups.Results After the intervention,the incidence of pressure ulcer(2.56%)and shoulder hand syndrome(5.13%)in the intervention group was lower than that in the routine group(20.51%)(20.51%)(P<0.05);After the intervention,the ADL score of the intervention group was(80.55±3.57)higher than that of the routine group(70.26±3.10)(P<0.05);Affer the intervention,the scores of edema,pain and joint activity in the intervention group were(0.69±0.09)(1.02±0.47)(2.10±0.57)lower than those in the routine group(1.19±0.04)(2.66±0.62)(3.55±1.02)(P<0.05).Conclusion The self-made elongated pillow combined with good limb placement can significantly reduce the incidence of pressure sores and shoulder and hand syndrome in patients after brain death,succes-sively improve the mobility of patients and promote physical recovery.
