大承气汤治疗急性胰腺炎作用机制的研究进展

摘　　要：急性胰腺炎是一种常见的急腹症,并发症高发,致死率较高,目前治疗以抗生素、补液等对症治疗为主。大承气汤是治疗急性胰腺炎的经典方剂,有通里攻下、清热解毒等功效,大量的动物实验和临床研究表明,大承气汤在治疗急性胰腺炎方面卓有成效。大黄素、厚朴酚、大黄酸、儿茶素是辅助治疗急性胰腺炎的主要有效单体。诸多实验证明大承气汤主要通过调控炎性因子(白细胞介素1β、白细胞介6、白细胞介8、白细胞介10、肿瘤坏死因子等)及相关信号通路、调控细胞自噬和凋亡、修复肠黏膜屏障、缓解氧化应激反应、调控胰腺细胞钙超载等途径发挥治疗急性胰腺炎的作用。本文通过查阅有关文献及相关研究成果,对大承气汤治疗急性胰腺炎的作用机制从以上方面进行论述,以期为大承气汤治疗急性胰腺炎的临床应用提供参考。Acute pancreatitis(AP)is a common acute abdomen with high incidence of complications and high Case fatality rate.At present,Symptomatic treatment mainly includes antibiotics and fluid replacement.Dachengqi decoction is a classic formula for clinical treatment of AP,with functions such as clearing the internal circulation,clearing heat,and detoxifying.A large number of animal experiments and clinical studies have shown that Dachengqi Tang is effective in treating AP.Among them,emodin,magnolol,rhein,and catechin are the main effective monomers for adjuvant treatment of AP.Many experiments have shown that Dachengqi Tang mainly regulates inflammatory factors(IL-1β,IL-6,IL-8,IL-10,TNF-α)and related signaling pathways(TLR/NF-κB,JAK2-STAT3,PI3K-AKT,etc.),regulate cell autophagy and apoptosis,repair intestinal mucosal barrier,alleviate oxidative stress response,and regulate pancreatic cell calcium overload to play a therapeutic role in AP.This article reviewed the mechanism of action of Dachengqi decoction in treating AP from the above aspects by reviewing relevant literature and research results,in order to provide references for the clinical application of Dachengqi decoction in treating AP.
