摘  要:目的对比评估Symbia T16型和NET632型SPECT仪进行心肌灌注显像(MPI)得到的图像质量与半定量参数的一致性。资料与方法回顾性分析2021年4—12月内蒙古医科大学附属医院37例患者上述2种机型上进行静息或负荷MPI资料,将所有图像处理得到半定量参数静息灌注总评分(SRS)、负荷灌注总评分(SSS)、舒张末期容积(EDV)、收缩末期容积(ESV)及左心室射血分数(LVEF)。采用Kappa值评估图像质量评分的一致性;运用组内相关系数(ICC)和Bland-Altman分析评估半定量参数的可重复性和一致性。结果NET632型SPECT MPI的图像分辨率、对左心室结构及心肌轮廓显示清晰程度较SymbiaT16型SPECT稍好。2种机型的图像质量评分一致性较好,差异有统计学意义(P<0.05)。37例患者进行静息MPI的SRS、EDV、ESV及LVEF的ICC分别为0.868、0.586、0.791及0.641,差异有统计学意义(P均<0.001)。27例患者进行负荷MPI的SSS、EDV、ESV及LVEF的ICC分别为0.822、0.783、0.931及0.719,差异有统计学意义(P均<0.001)。经BlandAltman分析,半定量参数差值在95%CI内。结论NET632型SPECT MPI的图像质量较SymbiaT16型稍好,且两者的图像质量评分与半定量参数一致性较好。Purpose To compare and evaluate the consistency of image quality and semi-quantitative parameters obtained from myocardial perfusion imaging(MPI)via SymbiaT16 and NET632 SPECT.Materials and Methods A total of 37 patients who underwent resting or load MPI on these two types of machines at the Affiliated Hospital of Inner Mongolia Medical University from April to December 2021 were retrospectively selected.All images were processed to obtain semi-quantitative parameters resting perfusion total score(SRS),total stress perfusion score(SSS),end-diastolic volume(EDV),end-systolic volume(ESV)and left ventricular ejection fraction(LVEF).The consistency of image quality scores was evaluated via Kappa values,and the repeatability and consistency of semi-quantitative parameters were evaluated via Intra-class correlation coefficient(ICC)and Bland-Altman analysis.Results NET632 SPECT MPI had slightly better image resolution and clearer display of left ventricular structure and myocardial contour compared to SymbiaT16 SPECT.The consistency of image quality scores between the two models was good,and the difference was statistically significant(P<0.05).The ICC of SRS,EDV,ESV and LVEF in 37 patients undergoing resting MPI were 0.868,0.586,0.791 and 0.641,respectively,with statistically significant differences(P<0.001).The ICC of SSS,EDV,ESV and LVEF in 27 patients underwent load MPI were 0.822,0.783,0.931 and 0.719,respectively,the differences were statistically significant(P<0.001).According to Bland Altman analysis,the difference in semi-quantitative parameters was within 95%CI.Conclusion The image quality of NET632 SPECT MPI is slightly better than that of SymbiaT16,with good consistent of their image quality scores and semi-quantitative parameters.
