身痛逐瘀汤加减配合针刺治疗腰椎间盘突出症合并坐骨神经痛患者的效果

摘　　要：目的:探究身痛逐瘀汤加减配合针刺对腰椎间盘突出症(LDH)合并坐骨神经痛患者的症状改善效果。方法:选取2022年1月—2023年1月宁谷镇卫生院LDH合并坐骨神经痛患者78例为研究对象,采用随机数表法等比例分为对照组、观察组,各39例。对照组实施身痛逐瘀汤加减治疗,观察组在对照组基础上加用针刺治疗。比较两组治疗效果、视觉模拟评分(VAS)、日本骨科协会相关量表(JOA)及Oswestry功能障碍指数问卷表(ODI)评分。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P=0.025);治疗后,两组VAS评分均降低,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组JOA评分均高于治疗前,ODI评分均低于治疗前,观察组JOA评分高于对照组,观察组ODI评分低于对照组,差异有统计学意义(P<0.001)。结论:身痛逐瘀汤加减联合针刺方式治疗LDH合并坐骨神经痛患者,可以提升治疗效果,改善患者腰椎功能,缓解疼痛感。Objective:To investigate the therapeutic effect of the addition of Shentong Zhuyu Decoction combined with acupunc⁃ture in patients with lumbar disc herniation(LDH)combined with sciatica.Methods:78 patients with LDH combined with sciatica in Ninggu Town Health Center were selected from January 2022 to January 2023,and they were divided into the control group and the observation group in equal proportions according to the randomized table method,39 cases in each group.The control group was treated with modified Shentong Zhuyu Decoction,while the observation group was treated with acupuncture on the basis of the control group.The therapeutic effect,visual analog score(VAS),Japanese Orthopaedic Association(JOA)and Oswestry Dysfunc⁃tion Index(ODI)scores were compared between the two groups.Results:The total effective rate of treatment in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.025);after treatment,the VAS scores of both groups were reduced,and the observation group was lower than that of the control group,and the difference was sta⁃tistically significant(P<0.05);after treatment,the JOA scores of both groups were higher than that of the pre-treatment period,while the ODI scores of both groups were lower than that of the pre-treatment period.the difference was statistically significant(P<0.001).Conclusion:The treatment of LDH combined with sciatica with Shentong Zhuyu Decoction combined with acupuncture can leads to a good therapeutic effect,improve the function of lumbar vertebrae,and alleviate the pain of the patients.
