摘 要:目的分析四川省遂宁市诺如病毒聚集性疫情中病毒的基因型及进化特征,为疫情防控提供参考。方法采集2020—2021年遂宁市3起诺如病毒聚集性疫情中病例、健康人(托幼机构教师和厨师)肛拭子标本及环境涂抹标本,采用实时荧光RT-PCR检测诺如病毒核酸,阳性标本进行基因同源性和系统进化分析。结果3起疫情共报告病例34例,波及3个班级132人,总罹患率为25.76%(34/132);采集到的19份病例标本中,13份阳性;5份教职工标本中1份阳性;20份环境标本中3份阳性。成功鉴定8份病例标本基因型,其中2份为GI.5[P4]型,6份为GII.2[P16]型,2起疫情GII.2[P16]型的基因同源性为100%。结论2020—2021年监测到GI.5[P4]、GII.2[P16]2种基因型诺如病毒在遂宁地区流行,系统分析及时预警,有助于学校聚集性疫情防控工作开展。Objective To investigate the genotype and evolutionary characteristics of norovirus in Suining City,Sichuan Province,to provide reference for epidemic prevention and control.Methods Anal swab specimens were collected from cases,healthy people(teachers and cooks in kindergartens),and environmental swabs from three clusters of norovirus outbreaks in Suining city from 2020 to 2021.Norovirus nucleic acid was detected by real-time RT-PCR,and the positive samples were analyzed for gene homology and phylogenetic evolution.Resμlts In total,34 cases were reported in the 3 outbreaks and involved 132 individuals in 3 classes,giving an overall prevalence of 25.76%(34/132).Thirteen of 19 case specimens were positive,1 out of 5 staff specimens were positive,and 3 out of 20 environmental specimens were positive.Genotypes of eight case specimens were successfully identified,of which two were GI.5[P4]type,six were GII.2[p16]type,and the gene homology between the two outbreaks GII.2[p16]type was 100%.Conclusions From 2020 to 2021,two genotypes of norovirus,GI.5[P4]and GII.2[P16],were detected in Suining.Systematic analysis and timely early warning are helpful for the prevention and control of cluster epidemics in schools.
