糖皮质激素治疗结核性胸膜炎的100例临床回顾性分析

摘　　要：目的:探究糖皮质激素对TP的应用效果。方法:依照纳入及排除标准选取我院2019年1月-2022年12月进行治疗的100例TP患者,随机平均将所有患者分为常规组(n=50)和激素组(n=50),两组TP患者在入院后均给予2HRZE/4HRE抗结核治疗和胸膜腔穿刺置管治疗,第一次放液低于600mL,之后1周进行3次抽胸液。激素组同时口服泼尼松片进行治疗,每天1次,每次30mg,在治疗2周后根据患者体温及胸腔积液情况调节用量,共治疗时间为6周,观察两组TP患者时间指标,疗效情况和不良反应出现情况。结果:激素组TP患者的总住院时间、发热总时间、胸腔积液持续时间显著小于常规组(P<0.05),治疗总有效数显著大于常规组(P<0.05),同时常规组和激素组TP患者发生不良反应的情况没有明显差异(P>0.05)。结论:糖皮质激素能有效改善TP患者的恢复情况,改善治疗效果,而且不会出现明显不良反应,可以在临床进行应用。Objective:To explore the application effect of glucocorticoid on TP.Methods:According to the inclusion and exclusion criteria,100 patients with TP treated in our hospital from January 2019 to December 2022 were randomLy divided into routine group(n=50)and hormone group(n=50).Two groups of TP patients were given 2HRZE/4HRE antituberculosis therapy and pleural puncture and catheterization after admission.The first drainage was less than 600mL,and then the thoracic fluid was pumped three times a week later.The hormone group was treated with oral prednisone tablets once a day,30mg each time.After 2 weeks of treatment,the dosage was adjusted according to the patient’s body temperature and pleural effusion,and the total treatment time was 6 weeks.The time index,curative effect and adverse reactions of TP patients in two groups were observed.Results:The total hospitalization time,total fever time and pleural effusion duration of TP patients in hormone group were significantly shorter than those in routine group(P<0.05),and the total effective number of treatment was significantly greater than that in routine group(P<0.05).At the same time,there was no significant difference in adverse reactions between routine group and hormone group(P>0.05).Conclusion:Glucocorticoid can effectively improve the recovery of TP patients and improve the therapeutic effect,and there will be no obvious adverse reactions,so it can be used in clinic.
