活血通脉稳斑汤联合增强型体外反搏治疗冠心病不稳定型心绞痛痰阻心脉证疗效

摘　　要：目的:探究活血通脉稳斑汤联合增强型体外反搏(EECP)治疗冠心病不稳定型心绞痛痰阻心脉证临床效果。方法:选取112例冠心病不稳定型心绞痛痰阻心脉证患者为研究对象,采用单双球法随机分为对照组、联合组,各56例。对照组给予EECP治疗,联合组给予活血通脉稳斑汤联合EECP治疗。比较两组治疗前、治疗4周后中医证候积分、心功能、血液流变学、血管内皮功能,记录两组患者治疗期间不良反应发生率。结果:治疗4周后,两组患者中医证候积分、纤维蛋白原、红细胞沉降率、全血黏度、血细胞比容、血栓素B2(TXB2)、内皮素-1(ET-1)水平降低,联合组低于对照组(均P<0.05)。治疗后,两组心排量、每搏输出量、二尖瓣口舒张早期血流速度峰值/左心房舒张晚期血流速度峰值、左心室射血分数、血管内皮生长因子、一氧化氮水平升高,且联合组高于对照组(均P<0.05)。治疗期间,联合组不良反应发生率为3.57%,对照组为5.36%,差异无统计学意义(P>0.05)。结论:活血通脉稳斑汤联合EECP可有效改善冠心病不稳定型心绞痛痰阻心脉证症状,增强患者心功能,改善血液流变学、血管内皮功能,且不增加不良反应。Objective:To explore effect of Huoxue Tongmai Wenban decoction combined with enhanced external counterpulsation(EECP)in treatment of unstable angina of coronary(UA)heart disease with Tanzu Xinmai syndrome.Methods:112 patients with UA Tanzu Xinmai admitted as research subjects were randomly divided into control group and combined group,56 cases in each group,by single-double ball method.The control group were given EECP,and the combined group treated with Huoxue Tongmai Wenban decoction combined with EECP.Before treatment and after 4 weeks of treatment,the TCM syndromes scores,cardiac function,hemorheology and vascular endothelial function were compared between the two groups.The incidence rates of adverse reactions during treatment were recorded in the two groups.Results:After 4 weeks of treatment,the TCM syndromes scores,fibrinogen,erythrocyte sedimentation rate,whole blood viscosity,hematocrit,TXB2,ET-1 in both groups were decreased,and the above indicators in the combined group were lower than those in the control group(all P<0.05).After treatment,CO,SV,E/A,LVEF,VEGF,NO in the two groups were enhanced,and the indicators were higher in the combined group than those in the control group(all P<0.05).During treatment,the toal incidence rate of adverse reactions was 3.57%in the combined group and 5.36%in the control group,difference not statistically significant(P>0.05).Conclusion:Huoxue Tongmai Wenban decoction combined with EECP can effectively improve the symptoms of unstable angina of coronary heart disease of Tanzu Xinmai syndrome,enhance the cardiac function,promote hemorheology and vascular endothelial function.It does not increase adverse reactions.
