摘  要:目的 探究难治性肺炎支原体肺炎(RMPP)患儿血清和肺泡灌洗液中社区获得性呼吸窘迫综合征毒素(CARDS TX)、肿瘤坏死因子-α(TNF-α)、血清淀粉样蛋白A(SAA)的表达水平和临床意义。方法 选择2021年2月至2023年2月郑州大学第一附属医院收治的接受支气管镜检的132例MPP患儿作为研究对象,其中47例RMPP患儿作为Ⅰ组,另外85例MPP患儿作为Ⅱ组,同期在医院门诊体检的80例健康儿童作为Ⅲ组。对比3组血清及Ⅰ组、Ⅱ组肺泡灌洗液的CARDS TX、TNF-α、SAA水平,绘制受试者工作特征(ROC)曲线评估其预测RMPP的效能。结果 Ⅰ组的血清及肺泡灌洗液CARDS TX、TNF-α、SAA水平高于Ⅱ组和Ⅲ组,且Ⅱ组高于Ⅲ组(P<0.05)。SAA曲线下面积(0.735)较TNF-α(0.711)、CARDS TX(0.705)高。结论 RMPP患儿CARDS TX、TNF-α、SAA表达水平较高,可能参与RMPP的致病过程,并可有效预测RMPP发生。Objective To investigate the expression levels of community-acquired respiratory distress syndrome toxin(CARDS TX),tumor necrosis factor-α(TNF-α)and serum amyloid A(SAA)in serum and alveolar lavage fluid of children with refractory Mycoplasma pneumoniae pneumonia(RMPP)and their clinical significance.Methods A total of 132 children with MPP admitted to the First Affiliated Hospital of Zhengzhou University for bronchoscopy from February 2021 to February 2023 were selected for the study,of which 47 children with RMPP served as groupⅠand 85 children with MPP served as groupⅡof the study,while 80 healthy children who had their medical checkups at the outpatient clinic of the hospital during the same period were selected as groupⅢ.The CARDS TX,TNF-αand SAA levels in serum of the three groups and alveolar lavage fluid in groupⅠand groupⅡwere compared,and plotted receiver operating characteristic(ROC)curve to assess their efficacy in predicting RMPP.Results The levels of CARDS TX,TNF-αand SAA in serum and alveolar lavage fluid of groupⅠwere higher than those of groupⅡand groupⅢ,and groupⅡwere higher than those of groupⅢ(P<0.05).The area under the curve of joint detection of SAA(0.735)was higher than TNF-α(0.711)and CARDS TX(0.705).Conclusion CARDS TX,TNF-αand SAA are highly expressed in children with RMPP,which may be involved in the pathogenic process of RMPP and may be effective in predicting RMPP occurrence.
