声带注射地塞米松联合嗓音矫治对声带息肉患者的疗效

摘　　要：目的 研究声带注射地塞米松联合嗓音矫治对声带息肉患者的疗效。方法 选取2020年2月至2022年1月在医院接受手术治疗的80例声带息肉患者,根据随机数字表法分为联合组与常规组,每组40例。两组均接受喉镜下显微切除术及术后嗓音矫治,其中联合组还接受地塞米松声带注射。比较治疗前后嗓音功能(最长发声时间、最高基频、最低音强)及发音质量(发音质量评分、自主发音能力评分),比较两组术后1 a复发率。结果 联合组声带充血消失时间、水肿消失时间、住院时间均短于常规组(P<0.05);治疗后两组最长发声时间、最高基频、发音质量评分、自主发音能力评分均升高,且联合组均高于常规组(P<0.05),最低音强均下降,且联合组低于常规组(P<0.05);两组复发率比较差异无统计学意义(P>0.05)。结论 声带息肉术后给予声带注射地塞米松联合嗓音矫治可获得理想效果,不仅能缩短康复时间,促进嗓音功能及发音质量的提升,还能在一定程度上减少复发。Objective To study the effect of vocal fold injection with dexamethasone combined with voice therapy in patients with vocal cord polyps.Methods A total of 80 patients with vocal cord polyps who underwent surgical treatment in the hospital from February 2020 to January 2022 were randomized divided into combination group and conventional group,with 40 patients in each group.Both groups underwent laryngoscopic microsurgery and postoperative voice therapy.On this basis,the combination group were treated with vocal fold injection with dexamethasone.Changes of voice function(maximum phonation time,highest frequency and minimum intensity)and voice quality(voice quality score and autonomous voice ability score)before and after treatment were recorded.The recurrence rate in 1 year after operation was evaluated.Results The disappearance time of vocal cord congestion,disappearance time of edema and hospital stay in the combination group were shorter than those in the conventional group(P<0.05).After treatment,maximum phonation time,highest frequency,voice quality scores and autonomous voice ability scores of two groups increased,and the combination group were higher than those of the conventional group(P<0.05),and minimum intensity of two groups decreased,and the combination group was lower than that of the conventional group(P<0.05).There was no statistically significant difference in recurrence rates between the two groups(P>0.05).Conclusion Implementing vocal fold injection with dexamethasone combined with voice therapy after vocal cord polyp surgery can achieve ideal results,which can not only shorten the rehabilitation time,promote the improvement of voice function and voice quality,but also reduce recurrence to a certain extent.
