摘　　要：目的 探讨牵复三步法治疗寰枢关节错缝所致眩晕的效果。方法 选取2020年10月至2023年1月医院收治的160例寰枢关节错缝所致眩晕患者,按随机数字表法分为观察组及对照组,各80例。对照组接受常规治疗,观察组接受牵复三步法治疗。比较两组的临床疗效、治疗前后的寰枢关节活动度、疼痛程度、眩晕评分、炎症因子指标[白介素-6(IL-6)和肿瘤坏死因子-α(TNF-α)]、椎动脉血流动力学指标[左右椎动脉平均血流速度(Vm)、收缩峰值流速(PSV)、搏动指数(PI)、阻力指数(RI)]。结果 治疗后两组Vm、PSV水平均提升,且观察组高于对照组(P<0.05)。观察组的治疗有效率(100.00%)高于对照组(85.00%)(P<0.05)。治疗后两组疼痛评分及IL-6、TNF-α水平均下降,且观察组低于对照组(P<0.05)。治疗后观察组和对照组的寰枢关节活动度、眩晕评分提升,且观察组高于对照组(P<0.05)。结论 牵复三步法治疗寰枢关节错缝所致眩晕患者的效果较好,能改善椎动脉血流动力学和眩晕症状,减轻机体炎症反应和颈部疼痛程度,促进错位恢复。Objective To explore the clinical efficacy of three-step traction and reduction in the treatment of vertigo caused by Atlanto-axial joint dislocation.Methods A total of 160 patients with vertigo caused by Atlanto-axial joint dislocation admitted the hospital from October 2020 to January 2023 were selected.According to the random number table method,they were divided into test group and control group,with 80 cases in each group.The control group accepted routine treatment,while the test group patients accepted traction and three-step treatment.The clinical efficacy,Atlanto-axial joint mobility,pain degree,vertigo score and inflammatory factor indicators[interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)],vertebral artery hemodynamics indexes[mean blood flow velocity(Vm),peak systolic velocity(PSV),pulsatility index(PI),resistance index(RI)of left and right vertebral arteries]before and after treatment were compared between the two groups.Results After treatment,the levels of Vm and PSV in test group and control group increased,and test group were higher than those in control group(P<0.05).The clinical efficacy of test group(100.00%)was higher than that of control group(85.00%)(P<0.05).After treatment,the pain score of Atlanto-axial joint,IL-6 and TNF-αin two groups decreased,and test group were lower than those in control group(P<0.05).After treatment,range of motion and the dizziness scores of two groups improved,and test group were higher than those of control group(P<0.05).Conclusion The clinical efficacy of three-step traction and reduction in the treatment of vertigo caused by Atlanto-axial joint dislocation is good,which can improve their vertebral artery hemodynamics and vertigo symptoms,reduce their inflammatory reaction and neck pain,and promote the recovery of dislocation.
