活血祛斑方治疗黄褐斑的临床疗效观察及其对血清MSH、ET-1、NO、VEGF的影响

摘　　要：目的:观察活血祛斑方治疗黄褐斑的临床疗效及对血清促黑素(MSH)、内皮素-1(ET-1)、一氧化氮(NO)、血管内皮生长因子(VEGF)的影响。方法:选取2019年4月—2022年4月在常山县人民医院医疗服务共同体和衢州市人民医院就诊的黄褐斑患者104例,根据抽签法随机分为对照组和观察组,各52例;另选取健康体检者30例做健康对照。对照组给予维生素C片口服治疗,观察组给予活血祛斑方治疗,治疗3个疗程评定疗效。治疗前后对皮损面积、皮损颜色、中医兼证症状进行评分,检测血清MSH、ET-1、NO、VEGF水平,统计6个月复发情况况。结果:治疗3个疗程后,观察组有效率较对照组提高(P<0.05)。与健康组相比,对照组和观察组治疗前MSH、ET-1、NO水平升高(P<0.05),VEGF水平降低(P<0.05);治疗后对照组和观察组较治疗前MSH、内皮素-1、NO水平均降低(P<0.05),VEGF水平升高(P<0.05);与对照组治疗后比较,观察组治疗后MSH、ET-1、NO水平下降(P<0.05),VEGF水平升高(P<0.05)。两组治疗后皮损面积、皮损颜色、中医兼证症状评分低于治疗前(均P<0.05);观察组较对照组皮损面积、皮损颜色、中医兼证症状评分降低明显(均P<0.05)。结论:活血祛斑方治疗黄褐斑临床疗效显著,能够改善患者临床症状,降低血清MSH、ET-1、NO水平,提高VEGF水平。Objective:To observe the clinical efficacy of Huoxue Quban Recipe in the treatment of chloasma and its impacts on serum melanotropin(MSH),endothelin-1(ET-1),nitric oxide(NO),and vascular endothelial growth factor(VEGF).Methods:104 patients with chloasma from Medical Service Community of Changshan County People’s Hospital and Quzhou People’s Hospital from April 2019 to April 2022 were randomly divided into control group and observation group,with 52 patients in each group;Another 30 healthy people were selected as health controls.The control group took vitamin C tablets,while the observation group was given Huoxue Quban Recipe.The therapeutic effect was evaluated after three courses of treatment.Before and after treatment,the area,color,and symptoms of traditional Chinese medicine(TCM)were scored,and serum levels of MSH,ET-1,NO,and VEGF were measured.the recurrence status during 6 months were also counted.Results:After 3 courses of treatment,the effective rate of the observation group was higher than that of the control group(P<0.05).Compared with the healthy group,the levels of MSH,ET-1,and NO in the control and observation groups were increased before treatment(P<0.05),while the level of VEGF was decreased(P<0.05);After treatment,the levels of MSH,ET-1,and NO in the control group and observation group were decreased compared to before treatment(P<0.05),while the level of VEGF was raised(P<0.05);Compared with the control group after treatment,the observation group showed a decrease in MSH,ET-1,and NO levels(P<0.05)and an increase in VEGF levels(P<0.05).After treatment,the area and color of skin lesions,as well as the score of TCM symptoms in both groups were lower than before treatment(all P<0.05);The observation group showed a significant decrease in skin lesion area,skin lesion color,and TCM syndrome score compared to the control group(all P<0.05).Conclusion:The Huoxue Quban Recipe has significant clinical efficacy in treating chloasma,which can improve patients’clinical symptoms,reduce serum MSH,ET-1,NO
