摘　　要：目的探讨右美托咪定不同给药时机对剖宫产术中寒战反应的影响。方法回顾性分析2021年4月至2023年4月于揭阳市揭东区妇幼保健院行择期子宫下段剖宫产术的60例产妇,根据不同给药时机将产妇分为麻醉后组(A组)和麻醉前组(B组),每组30例。两组产妇均进行腰硬联合麻醉,A组产妇在腰硬联合麻醉操作后即刻缓慢静脉滴注右美托咪定,B组产妇在腰硬联合麻醉前15 min缓慢静脉滴注右美托咪定。比较两组产妇麻醉前后、切皮时、手术结束时的平均动脉压和心率,记录两组产妇寒战和不良反应发生情况。结果在麻醉前、麻醉后、切皮时、手术结束时两组产妇平均动脉压和心率比较,差异均无统计学意义(P>0.05)。B组产妇寒战发生率低于A组(P<0.05)。两组均未发生不良反应。结论子宫下段剖宫产术中在腰硬联合麻醉操作前静脉滴注右美托咪定预防寒战反应效果更佳。Objective To investigate the effect of different administration timing of Dexmedetomidine on shivering response during cesarean section.Methods A retrospective analysis was conducted on 60 puerperas undergoing elective lower uterine segment cesarean section in Jieyang Jiedong Maternal and Child Health Hospital from April 2021 to April 2023.The puerperas were divided into postanesthetic group(group A)and preanesthesia group(group B)according to different administration timing,with 30 cases in each group.Both groups received combined spinal-epidural anesthesia.The group A received slow intravenous infusion of Dexmedetomidine immediately after the combined spinal-epidural anesthesia,while the group B received slow intravenous infusion of Dexmedetomidine 15 minutes before the combined spinal-epidural anesthesia.The mean arterial pressure and heart rate before and after anesthesia,at the time of skin incision and the end of the operation were compared between the two groups,and the occurrence of shivering and adverse reactions were recorded in the two groups.Results There were no significant differences in mean arterial pressure and heart rate between the two groups before and after anesthesia,at the time of skin incision and the end of the operation(P>0.05).The incidence of shivering in group B was lower than that in group A(P<0.05).No adverse reactions occurred in either group.Conclusion Intravenous infusion of Dexmedetomidine before the combined spinal-epidural anesthesia in lower uterine segment cesarean section is more effective in preventing shivering response.
