痔上黏膜结扎术联合痔切除半闭锁缝合治疗混合痔的临床疗效分析

摘　　要：目的:探讨痔上黏膜结扎术联合痔切除半闭锁缝合治疗混合痔的临床疗效。方法:选择2022年3~12月我科收治的62例混合痔患者,随机分为对照组和观察组,各31例,对照组采用混合痔外剥内扎术治疗,观察组采用痔上黏膜结扎术联合痔切除半闭锁缝合治疗。比较2组临床治疗总体疗效、术中出血量、手术时间、创面愈合时间、住院时间,术后首次排便时及不同时间静息时疼痛程度,以及术后并发症发生情况。结果:2组总有效率及治愈率比较差异均无统计学意义(P>0.05)。观察组手术时间长于对照组(P<0.05),术中出血量小于对照组(P<0.05),创面愈合时间和住院时间均短于对照组(P<0.05)。观察组术后首次排便时及术后12 h、24 h、48 h、第3天、第7天疼痛VAS评分均低于对照组(P<0.05)。观察组术后出血、肛门狭窄、肛门坠胀等并发症总发生率明显低于对照组(P<0.05)。结论:痔上黏膜结扎术联合痔切除半闭锁缝合治疗混合痔疗效确切,与外剥内扎术相比,术后并发症更少,患者痛苦更小、恢复更快,不失为一种理想的手术方式。Objective To explore the clinical efficacy of hemorrhoid mucosal ligation combined with hemorrhoidectomy with semi closed suture in the treatment of mixed hemorrhoids(MH).Methods 62 patients with MH admitted to author's hospital from March to December 2022 were randomly divided into control group(n=31,treated with mixed hemorrhoid external dissection and internal ligation)and observation group(n=31,treated with hemorrhoid mucosal ligation combined with Hemorrhoidectomy with semi closed suture),then compared the overall clinical treatment efficacy,intraoperative bleeding volume,surgical time,wound healing time,hospital stay,pain degree at first bowel movement and at different resting times after surgery,and the occurrence of postoperative complications between the two groups.Results There was no statistically significant difference in the total effective rate and cure rate between the two groups(P>0.05).The observation group had longer surgical time than the control group(P<O.05),less intraoperative bleeding,shorter wound healing time and hospital stay than the control group(P<0.05).The observation group had lower pain VAS scores at the first postoperative defecation,as well as at 12 h,24 h,48 h,3 rd and 7 th days after surgery compared to the control group(P<0.05).The total incidence of postoperative complications such as bleeding,anal stenosis,and anal tenesmus in the observation group was significantly lower than that in the control group(P<0.05).Conclusion The combination of hemorrhoid mucosal ligation and hemorrhoidectomy with semi closed suture is effective in the treatment of mixed hemorrhoids.Compared with external dissection and internal ligation,it has fewer postoperative complications,less patient pain,and faster recovery,making it an ideal surgical method.
