临床药师参与1例天疱疮患者疑似感染毛霉菌病的治疗体会

摘　　要：目的探讨毛霉菌病的治疗方案。方法临床药师通过1例有天疱疮基础疾病伴免疫功能受损疑似感染肺毛霉菌病合并皮肤型毛霉菌病患者,根据其基础状态调整抗感染治疗方案。结果患者使用两性霉素B脂质体(锋克松)不耐受,出现包括肾功能损害及其它严重不良反应,停用两性霉素B脂质体(锋克松)换用泊沙康唑口服混悬液不能控制病情,重新启用小剂量两性霉素B脂质体(安浮特克)联合泊沙康唑口服混悬液治疗后,病情好转出院。结论对于出现肾功能损害的毛霉菌病患者可以用小剂量两性霉素B脂质体联合泊沙康唑治疗。OBJECTIVE To investigate the treatment of mucormycosis.METHODS A patient with pemphigus underlying disease and impaired immune function suspected of pulmonary mucormycosis combined with cutaneous mucormycosis was treated by clinical pharmacist according to the underlying status.RESULTS The patient was intolerant to amphotericin B liposomes(Foxonazole),which resulted in renal function impairment and other serious adverse reactions.Discontinuation of amphotericin B liposomes(Foxonazole)and replacement of Posaconazole oral suspension could not control the disease.After re-use of low-dose amphotericin B liposomes(Ambutek)combined with posaconazole oral suspension,the condition improved and the patient was discharged.CONCLUSION Low dose amphotericin B liposome combined with posaconazole can be used to treat mucormycosis with renal function impairment.
