利水中药干预高血压病致心房重构的研究进展

摘　　要：利水中药有利尿作用,可降低血压、减轻心脏负荷,其提取物亦可通过调节肾素-血管紧张素-醛固酮系统(renin-angio-tensin-aldosterone system,RAAS)、抑制促纤维化因子作用、抗氧化、抗炎等途径改善高血压病致心房重构,与西药相比,具有不良反应较小、较安全的优势。目前,针对利水中药及其提取物干预高血压病致心房重构的研究较为匮乏,且存在如下不足:相关研究多为动物及细胞实验,缺乏中医辨证施治研究,有待加强中医思想及理论的指导;对利水中药及其提取物的药理作用及机制研究不够完善,需进一步加强药效动力学、药代动力学和药物毒理学研究;此外,利水中药及其提取物对高血压病致心房重构的治疗尚缺乏有力的证据支持,需进行大样本、多中心的临床试验研究,为高血压病心房重构的治疗提供参考。Chinese herbal medicine for diuresis reduces blood pressure and heart load mainly through alleviate water retention.Its extract can also improve atrial remodeling caused by hypertension by regulating RAAS,inhibiting the role of pro fibrosis factors,antioxidant,anti-inflammatory and other ways.Compared with western medicine,it has the advantages of less adverse reactions and safety.At present,there is relatively little research on some diuretic Chinese herbs and extracts,and there are the following shortcomings:the relevant research on the intervention of diuretic Chinese herbs in atrial remodeling caused by hypertension is mostly animal and cell experiments,and there is a lack of research on traditional Chinese medicine syndrome differentiation and treatment.It is necessary to strengthen the guidance of traditional Chinese medicine thinking and theory;The research on the pharmacological effects and mechanisms of Chinese herbal medicines and their extracts for promoting diuresis is not comprehensive enough,and further efforts need to be made to strengthen the research on pharmacodynamics,pharmacokinetics and pharmacotoxicology;In addition,there is still a lack of strong evidence to support the treatment of atrial remodeling caused by hypertension with Chinese herbal medicine and its extracts.Large sample,multi-center clinical trials are needed to provide reference for the treatment of atrial remodeling in hypertension.
