黄连紫草膏联合红外线烤灯照射对糖尿病足溃疡创面愈合及血清学指标的影响

摘　　要：目的观察黄连紫草膏联合红外线烤灯照射对糖尿病足溃疡(DFU)创面愈合情况及血清学指标的影响。方法选取2020年1月至2021年12月湖北省武汉市中医医院收治的128例DFU患者为研究对象,采用掷硬币法分为对照组与观察组,每组64例。2组均进行常规治疗,对照组在此基础上联合红外线烤灯照射治疗,观察组在对照组基础上联合黄连紫草膏治疗,2组均连续治疗3周。比较2组患者临床疗效、创面愈合情况、血清学指标[肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、5-羟色胺],广义估计方程分析2组之间创面愈合率,并比较治疗期间不良反应发生情况。结果治疗3周后,观察组总有效率高于对照组,差异有统计学意义[87.5%(56/64)比71.9%(46/64),χ^(2)=4.827,P=0.028]。基于广义估计方程分析:治疗2、3周时,2组创面愈合率较治疗1周时均升高,且观察组治疗1、2、3周时创面愈合率均高于对照组(P<0.05)。治疗3周时,2组血清TNF-α、IL-6水平均低于治疗前,5-羟色胺水平均高于治疗前,且观察组血清TNF-α、IL-6水平均低于对照组,5-羟色胺水平高于对照组(均P<0.05)。2组治疗期间均未发生明显不良反应。结论黄连紫草膏联合红外线烤灯照射治疗可有效促进DFU的创面愈合,调节血清TNF-α、IL-6、5-羟色胺水平,减轻疼痛,且安全性较高。Objective To observe the effects of Huanglian Zicao ointment combined with infrared lamp irradiation on wound healing and serological indexes of diabetic foot ulcer(DFU).Methods From January 2020 to December 2021,128 patients with DFU admitted to Wuhan Hospital of Traditional Chinese Medicine,Hubei Province were selected.They were divided into control group and observation group by coin tossing method,with 64 cases in each group.Both groups received routine treatment.On this basis,the control group was treated with infrared lamp irradiation,and the observation group was treated with Huanglian Zicao ointment based on the control group.Both groups were treated for 3 weeks.The clinical efficacy,wound healing,serological indexes[tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),5-hydroxytryptamine(5-HT)]were compared between the two groups.Wound healing rate was analyzed by generalized estimating equation,and the occurrence of adverse reactions during treatment of patients in both groups were observed.Results After 3 weeks of treatment,the total clinical effective rate of the observation group was higher than that of the control group[87.5%(56/64)vs 71.9%(46/64),χ^(2)=4.827,P=0.028].Based on the analysis of generalized estimation equation,the results showed that the wound healing rate of both groups increased at 2 and 3 weeks of treatment than that at 1 week of treatment,and the wound healing rate of the observation group was higher than that of the control group at 1,2 and 3 weeks of treatment(P<0.05).At 3 weeks of treatment,the levels of serum TNF-αand IL-6 in both groups decreased than those before treatment,and the levels of 5-HT increased than those before treatment;the levels of serum TNF-αand IL-6 in the observation group were lower than those in the control group,and the level of 5-HT was higher than that in the control group(all P<0.05).No obvious adverse reactions occurred in both groups during treatment.Conclusion Infrared lamp irradiation combined with Huanglian Zicao ointment can effectively promot
