水蛭酊湿热敷联合TDP照射促自体动静脉内瘘成熟临床观察

摘　　要：目的观察水蛭酊湿热敷配合TDP照射对早期自体动静脉内瘘成熟的影响。方法选取2020年6月—2021年12月收治的80例行自体动静脉内瘘患者,随机分为对照组和试验组,各40例。对照组术后采用常规护理和功能训练,试验组在对照组基础上用水蛭酊湿热敷联合TDP照射治疗,2组均治疗8周后,观察2组恢复情况。结果试验组术后4周及8周的头静脉内径、血流量改善情况均优于对照组(P<0.05);术后8周的动静脉内瘘成熟率、使用率均高于对照组(P<0.05);试验组治疗后NO及ET-1改善情况均优于试验组(P<0.01)。结论水蛭酊湿热敷配合TDP照射对早期自体动静脉内瘘干预可促进内瘘成熟,值得临床推广。Objective To observe the effect of moist heat application of leech tincture combined with TDP irradiation on the maturation of early arteriovenous fistulae.Methods Eighty patients who underwent arteriovenous fistulae from June 2020 to December 2021 were selected and randomly divided into control group and experimental group,with 40 cases in each.The control group was treated with routine nursing and functional training after surgery,and the experimental group was treated with wet hot compress of leech TDP irradiation on the basis of the control group.After 8 weeks of treatment,the recovery of the two groups was observed.Results The improvement of internal diameter and blood flow of cephalic vein in experimental group at 4 and 8 weeks after operation were better than those in control group(P<0.05).The maturity rate and utilization rate of arteriovenous fistula at 8 weeks after operation were higher than those in control group(P<0.05).The improvement of NO and ET-1 in experimental group after treatment was better than that in experimental group(P<0.01).Conclusion Moist heat application of leech tincture combined with TDP irradiation on early arteriovenous fistulae intervention can promote internal fistula maturation,and is worthy of clinical promotion.
