选择性痔上黏膜切除术联合痔动脉结扎术治疗重度混合痔合并直肠黏膜内脱垂的临床效果

摘　　要：目的:分析选择性痔上黏膜切除术联合痔动脉结扎术治疗重度混合痔合并直肠黏膜内脱垂的临床效果。方法:选取2021年11月—2022年11月江门市新会区中医院收治的60例重度混合痔合并直肠黏膜内脱垂患者。根据随机数表法将其分为对照组和研究组,各30例。对照组给予选择性痔上黏膜切除术,研究组给予选择性痔上黏膜切除术联合痔动脉结扎术。比较两组围手术期指标,术后1 d、2 d、3 d疼痛程度,临床疗效,并发症及复发情况。结果:两组手术时间及术中出血量比较,差异无统计学意义(P>0.05)。术后1 d、2 d、3 d,两组视觉模拟评分法(visual analogue scale,VAS)评分比较,差异无统计学意义(P>0.05)。两组总有效率比较,差异无统计学意义(P>0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。研究组复发率低于对照组,差异有统计学意义(P<0.05)。结论:重度混合痔合并直肠黏膜内脱垂患者采用选择性痔上黏膜切除术与选择性痔上黏膜切除术联合痔动脉结扎术均可以获得满意疗效,但选择性痔上黏膜切除术联合痔动脉结扎术治疗可降低术后复发率,且未加重患者术后疼痛程度,未增加并发症。Objective:To analyze the clinical effect of selective hemorrhoidal superior mucosal resection combined with hemorrhoidal artery ligation in the treatment of severe mixed hemorrhoids complicated with rectal intramucosal prolapse.Method:Sixty patients with severe mixed hemorrhoids complicated with rectal intramucosal prolapse admitted to Jiangmen Xinhui District Hospital of Traditional Chinese Medicine from November 2021 to November 2022 were selected.According to the method of random number table,they were divided into control group and study group,30 cases in each group.The control group was given selective hemorrhoidal superior mucosal resection,and the study group was given selective hemorrhoidal superior mucosal resection combined with hemorrhoidal artery ligation.Perioperative indexes,pain degree at 1 d,2 d and 3 d after surgery,clinical efficacy,complications and recurrence were compared between the two groups.Result:There were no significant differences in surgery time and intraoperative bleeding loss volume between the two groups(P>0.05).There were no significant differences in visual analogue scale(VAS)scores between the two groups on 1 d,2 d and 3 d after surgery(P>0.05).There was no significant difference in total effective rate between the two groups(P>0.05).There was no significant difference in the incidence of postoperative complications between the two groups(P>0.05).The recurrence rate of the study group was lower than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:In patients with severe mixed hemorrhoids complicated with rectal intramucosal prolapse,both selective hemorrhoidal superior mucosal resection and selective hemorrhoidal superior mucosal resection combined with hemorrhoidal artery ligation can achieve satisfactory results.However,selective hemorrhoidal superior mucosal resection combined with hemorrhoidal artery ligation treatmrnt can reduce postoperative recurrence rate,without increasing postoperative pain degree and complications of pati
