二三九四中西医结合慢病管理模式在老年高血压病的应用

摘　　要：目的探求高血压病慢性病管理的有效方法,通过研究将“二三九四”中西医结合慢病管理模式推广运用。方法将符合纳入标准的93例研究对象依据就诊顺序随机分为观察组(47例)和对照组(46例),对照组采用常规西医治疗,观察组在原有常规西医治疗的基础上采用二三九四模式,半年后对比2组患者血压控制情况及总有效率。结果干预后,观察组较对照组收缩压及舒张压更低,差异有统计学意义(P<0.05);观察组总有效率为89.36%(42/47),明显高于对照组的71.74%(33/46),差异有统计学意义(P<0.05)。结论二三九四模式为老年高血压病患者提供了具体可靠的慢病管理措施,起到了降低血压的效果,值得推广使用。Objective To explore the effective method of chronic disease management of hypertension,and to promote the application of the“2394”model combined with chronic disease management model.Methods Ninety-three cases who met the inclusion criteria were randomly divided into the observation group of 47 cases and the control group of 46 cases according to the order of consultation.The control group was treated with conventional Western medicine,while the observation group was treated with 2394model based on conventional Western medicine.The blood pressure control and total efficiency of the two groups were compared after six months.Results After intervention,the systolic and diastolic blood pressure in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).The total effective rate of the observation group was 89.36%(42/47),which was significantly higher than that of the control group(71.74%,33/46),and the difference was statistically significant(P<0.05).Conclusion The 2394 model provides specific and reliable chronic disease management measures for elderly patients with hypertension,and is worth promoting for its effectiveness in lowering blood pressure.
