多期MSCT增强扫描联合三维重建技术用于结肠癌术前评估的临床价值分析

摘　　要：目的:探究多期多层螺旋CT (MSCT)增强扫描联合三维重建技术于结肠癌术前评估的应用价值。方法:选取2020年7月—2022年9月平阴县人民医院收治的疑似结肠癌患者90例,均行多期MSCT增强扫描及三维重建成像检查。分析多期MSCT增强扫描的术前影像学征象以病理学诊断结果为金标准,分析多期MSCT增强扫描及联合检查在结肠癌术前评估中的应用价值。结果90例疑似结肠癌患者,经病理组织学检查明确为结肠癌68例,结肠良性病变22例,多期MSCT增强扫描联合三维重建技术在结肠癌术前诊断中的灵敏度、准确率及阴性预测值均高于多期MSCT增强扫描(P<0.05);两种方法诊断特异度及阳性预测值比较差异不显著(P> 0.05)。多期MSCT增强扫描结果与金标准一致性不佳(Kappa=0.391);多期MSCT增强扫描联合三维重建技术诊断结果与金标准一致性良好(Kappa=0.883);结肠癌最常见的病变部位为升结肠,以溃疡型病变及肠壁增厚表现发生率最高,经影像学检查可见邻近脂肪间隙较为模糊,中度强化(21~40 HU)发生率较高。结论:结肠癌术前评估采用多期MSCT增强扫描联合三维重建技术有助于判断病情严重程度,其诊断结果与“金标准”一致性较强,临床应用价值较高,可为临床诊疗提供有效参考依据。Objective To explore the value of multi-phase multi-slice spiral CT(MSCT)enhanced scanning combined with iterative reconstruction in preoperative evaluation of colon cancer.Methods A total of 90 suspected colon cancer patients admitted to Pingyin County People's Hospital from July 2020 to September 2022 were selected,all of whom underwent multi-phase MSCT enhanced scanning and three-dimensional reconstruction imaging.The preoperative imaging features of multi-phase MSCT enhanced scan were analyzed.Taking pathological diagnosis as the gold standard,the value of multi-phase MSCT enhanced scan and combined examination in preoperative evaluation of colon cancer was analyzed.Results Among 90 suspected patients with colon cancer,68 cases were confirmed as colon cancer and 22 cases were confirmed as benign lesions by histomatology.The sensitivity,accuracy and negative predictive value of multi-phase MSCT enhanced scanning combined with 3D reconstruction were higher than those of multi-phase MSCT enhanced scanning in preoperative diagnosis of colon cancer(P<0.05);There were no significant differences in specificity and positive predictive value between the two methods(P>0.05).The consistency of multi-phase MSCT enhanced scan and gold standard was poor(Kappa=0.391).The consistency of multi-phase MSCT enhanced scanning combined with 3D reconstruction and gold standard was good(Kappa=0.883).The most common lesion site of colon cancer is the ascending colon,with the highest incidence of ulcerative lesions and intestinal wall thickening.Imaging examination showed that the adjacent fat space was blurred,and the incidence of moderate enhancement(21~40HU)was higher.Conclusion The combination of multi-phase MSCT enhanced scanning and Iterative reconstruction technology in preoperative evaluation of colon cancer is helpful to judge the severity of the disease.Its diagnostic results are in strong consistency with the"gold standard"and have high clinical application value,which can provide effective reference for clinical diagnosis
