探析舒芬太尼复合布比卡因腰硬联合麻醉对髋部手术患者血流动力学影响

摘　　要：目的探讨舒芬太尼复合布比卡因腰硬联合麻醉对髋部手术患者血流动力学影响。方法将2020年1月-2022年6月在本院骨科治疗的40例髋部手术患者随机分为两组,均行腰硬联合麻醉,对照组单用布比卡因,观察组使用舒芬太尼复合布比卡因,对比两组的麻醉各指标、麻醉效果、血流动力学指标、不良反应。结果观察组感觉阻滞起效时间、运动阻滞起效时间、达T10时间、至最高平面时间均短于对照组,而维持镇痛时间长于对照组(P<0.05);观察组麻醉优良率明显高于对照组(P<0.05);观察组麻醉5min、10min、20min、30min的MAP、HR均高于对照组(P<0.05);观察组恶心呕吐、头晕、低血压、寒战等不良反应发生率低于对照组(P<0.05)。结论舒芬太尼复合布比卡因腰硬联合麻醉对髋部手术的效果显著,感觉及运动阻滞迅速,镇痛维持时间长,能稳定血流动力学,且安全性高。Objective To investigate effect of sufentanil combined with bupivacaine on hemodynamics of hip surgery patients.Methods The paper chose 40 cases of hip surgery patients in our orthopaedics department from January 2020 to June 2022,and divided them into two groups randomly.All patients were treated with combined spinal epidural anesthesia.Control group was treated with bupivacaine only and observation group with sufentanil combined with bupivacaine.Anesthesia indexes,anesthesia effect,hemodynamic indexes and adverse reactions between two groups were compared.Results Onset time of sensory block,motor block,reaching T10 and reaching the highest plane in observation group was shorter than control group,while duration of maintaining analgesia was longer than control group(P<0.05).Excellent and good anesthesia rate in observation group was significantly higher than control group(P<0.05).MAP and HR of observation group under anesthesia for 5 minutes,10 minutes,20 minutes and 30 minutes was higher than control group(P<0.05).Incidence of nausea,vomiting,dizziness,Hypotension,chills and other adverse reactions in observation group was lower than control group(P<0.05).Conclusion Combined spinal epidural anesthesia with sufentanil and bupivacaine can achieve significant effect in hip surgery,with rapid sensory and motor block,long duration of analgesia,stable hemodynamics and high safety.
