悦脾汤对孤独症谱系障碍患儿行为表现及胃肠症状的疗效分析

摘　　要：目的:观察悦脾汤治疗孤独症谱系障碍(ASD)共患脾胃不和患儿的临床疗效。方法:采用随机数字表法将60例共患脾胃不和ASD患儿分为对照组和治疗组,各30例。对照组予常规康复治疗,每日训练2 h,每周5次,连续3个月为1个疗程;治疗组在对照组基础上口服悦脾汤加减,1剂/d,每周5剂,连续3个月。比较两组治疗后儿童孤独症行为量表(ABC)、Gesell发育量表评分、胃肠疾病中医症状积分,并评定临床疗效。结果:治疗组总有效率为72.41%(21/29),对照组总有效率为55.17%(16/29),两组总有效率比较,差异有统计学意义(P<0.05)。治疗后,两组各量表评分均较治疗前改善,治疗组患儿感觉、交往、自理、语言能区评分降低程度大于对照组(P<0.05),精细运动、适应能力、语言、个人社交方面的改善均优于对照组(P<0.01),且中医症状积分显著低于对照组(P<0.01)。结论:悦脾汤治疗孤独症谱系障碍(ASD)共患脾胃不和患儿具有明显优势,可有效改善患儿行为表现和胃肠症状。Objective:To observe the clinical efficacy of Yuepi decoction on children with autism spectrum disorder(ASD)co-existing with spleen and stomach disharmony.Methods:Totally 60 ASD children with spleen and stomach disharmony were randomly divided into control and treatment groups,with 30 cases in each group.The control group received routine rehabilitation treatment,training 2 hours a day,5 times a week,for 3 months as a course of treatment.On the basis of the control group,the treatment group was treated with Yuepi decoction orally,one dose a day,5 doses a week,for 3 months.The scores of autism Behavior Scale(ABC),Gesell Developmental Scale and TCM syndrome score of gastrointestinal diseases were compared between the two groups after treatment,and the clinical efficacy was evaluated.Results:The total effective rate was 72.41%(21/29)in treatment group and 55.17%(16/29)in control group.There was significant difference in total effective rate between the two groups(P<0.05).After treatment,the scores of all scales were improved in the two groups compared with those before treatment.The scores of sensation,communication,self-care and language ability in the treatment group decreased more than those in the control group(P<0.05).The improvement of fine motor,adaptive ability,language and personal social interaction in the treatment group was better than that in the control group(P<0.01),and the treatment group showed lower TCM syndrome score than control group(P<0.01).Conclusion:Yuepi decoction is effective in the treatment of ASD children with spleen and stomach disharmony.And it can effectively improve the behavior and gastrointestinal symptoms.
