温阳益气化瘀利水法在心肾综合征的应用

摘　　要：心力衰竭是目前心血管内科疾病中最常见的疾病之一,目前西医对心力衰竭的纠正只能以改善症状为主,对远期的预后效果欠佳,心肾综合征是在心力衰竭的基础上出现肾功能不全,其早期治疗极为关键,当出现肾功能不全时,易导致利尿剂抵抗,加重水钠潴留,进而加重心衰,其后导致肾灌注进一步降低,肾功能衰竭进一步加重,从而形成恶性循环。中医学从辨证论治出发,根据患者的中医证候,综合舌苔、脉象,总结出心肾综合征的中医基本病机为“心肾阳虚”与“血瘀水停”,并以“益气温阳、活血利水”为治疗原则,将中西医治疗进一步有效结合,为心肾综合征的治疗提供了新靶点、新方案,有较好的治疗效果,值得在临床推广。Heart failure is one of the most common cardiovascular diseases today,the western medicine for the correct only to improve the symptoms of heart failure,poor effect on long-term prognosis,heart kidney syndrome is on the basis of heart failure in renal insufficiency,its early treatment is very critical,when renal function is not secure,easy to cause the diuretic resistance,increase water sodium retention,thus aggravating heart failure,then lead to further reduce renal perfusion,renal failure further,thus forming a vicious circle.According to patients' symptoms of traditional Chinese medicine,integrated coating on the tongue and pulse condition,the basic pathogenesis of heart kidney syndrome of traditional Chinese medicine “heart kidney yang deficiency” and “blood stasis and water retention”,and “benefiting qi and warming yang,promoting blood and benefiting water” as therapeutic principles,the combination of traditional Chinese and Western medicine provided a new target for the treatment of heart kidney syndrome,the new scheme,has good therapeutic effect in clinic,and is worth popularizing in clinical practice.
