预防应用抗菌药物对骨科内固定物Ⅰ类切口手术部位感染的效果分析

摘　　要：目的探讨骨科内固定物Ⅰ类切口手术围手术期预防应用抗菌药物对手术部位感染的影响,为规范预防用药提供依据。方法采用回顾性与随访调查方法,将某院2018年1月1日—2022年8月31日骨科内固定物Ⅰ类切口手术出院病例329例分为预防应用抗菌药物组与未预防应用抗菌药物组,比较两组患者手术部位感染和术后住院天数差异。结果骨科内固定物Ⅰ类切口手术预防应用抗菌药物比例为51.36%;未预防应用抗菌药物组手术部位感染率为0.63%,预防应用抗菌药物组手术部位感染率为0.59%,两组差异无统计学意义(P>0.05);未预防应用抗菌药物组与预防应用抗菌药物组的术后住院天数差异也无统计学意义(Z=-0.729,P=0.466)。结论在强化医院感染防控措施基础上,骨科内固定物Ⅰ类切口手术不推荐常规给予抗菌药物预防感染。Objective To investigate the effect of prophylactic use of antibiotics on surgical site infection during perioperative period of Class Ⅰ incision of orthopedic internal fixation,and to provide a basis for standardized prophylactic medication.Methods Using retrospective and follow-up survey methods,329 patients with Class Ⅰ incision surgery of orthopedic internal fixation discharged from our hospital from January 1,2018 to August 31,2022 were divided into group of prophylactic use of antibiotics and group of non-prophylactic use of antibiotics.The differences in surgical site infections and postoperative hospitalization days between the two groups were compared.Results The proportion of prophylactic use of antibiotics in Class Ⅰ incision surgery of orthopedic internal fixation was 51.36%;the infection rate of surgical site in the group of non-prophylactic use of antibiotics was 0.63%,while the infection rate of surgical site in the group of prophylactic use of antibiotics was 0.59%,and there was no statistically significant difference between the two groups(P>0.05);there was also no statistically significant difference in postoperative hospitalization days between the two groups(Z=-0.729,P=0.466).Conclusion On the basis of strengthening the prevention and control measures of nosocomial infection,it is not recommended to routinely administer antibiotics to prevent infection during Class Ⅰ incision surgery of orthopedic internal fixation.
