摘　　要：目的:探讨骨髓间充质干细胞移植促进跨区皮瓣术后血管新生的作用。方法:通过全骨髓差异贴壁法获取骨髓间充质干细胞,进行经90%融合后消化传代扩增,至第3代后行CD29^(+)、CD90^(+)、CD34^(-)以及CD45^(-)细胞免疫表型鉴定后待检,选取40只体质量为300~350g的SD大鼠作为研究对象,并按照数字随机法进行分组,每组各20只,对照组切取大鼠背部一侧跨3个区皮瓣后缝回原处,观察组切取大鼠背部一侧跨3个区皮瓣,缝回原处1h后,以1mL/100g的标准注射骨髓间充质干细胞悬液于尾静脉,浓度:1×10^(6)个/mL。观察两组SD大鼠1w后的皮瓣坏死率、choke血管的管径。结果:第3代SD大鼠分离培养的骨髓间充质干细胞中CD29、CD34、CD90、CD45的阳性率分别为98.2%,、0.82%、93.5%和2.22%。术后1wSD大鼠对照组皮瓣坏死率为(29.37±4.08)%,choke血管的管径为(182.17±22.24)μm,观察组SD大鼠皮瓣坏死率为(10.29±3.24)%,choke血管的管径为(225.34±22.19)μm,两组存在显著差异(P<0.05)。结论:骨髓间充质干细胞移植能够有效提高皮瓣新生率,并具有促进choke血管扩张的作用。Objective:To investigate the effect of bone marrow mesenchymal stem cell transplantation on angiogenesis after cross-regional skin flap surgery.Methods:Bone marrow mesenchymal stem cells were obtained by whole bone marrow differential adherence method,After 90%fusion,digestion,passage and amplification were carried out,After the third generation,CD29^(+),CD90^(+),CD34^(-),And CD45^(-)cell immunophenotype identification to be detected,40 SD rats weighing 300~350g were selected as the research objects,The rats were randomly divided into groups according to the number method,20 rats in each group.The control group cut the skin flap across three regions on the back side of the rats and then sewed it back to the original place.The observation group cut the skin flap across three regions on the back side of the rats and injected the bone marrow mesenchymal stem cells suspension into the tail vein with the standard of 1mL/100g at the concentration of 1×10^(6)/mL for 1 hour after sewing it back to the original place.The necrosis rate of skin flap and the diameter of choke blood vessel of SD rats in two groups were observed after 1 week of trunk.Results:The positive rates of CD29,CD34,CD90 and CD45 were 98.2%,0.82%,93.5%and 2.22%,respectively.The necrosis rate of flap was(29.37±4.08)%,the diameter of choke vessel was(182.17±22.24)μm in control group and(10.29±3.24)%,the diameter of choke vessel was(225.34±22.19)μm in observation group,there was significant difference between the two groups(P<0.05).Conclusion:Bone marrow mesenchymal stem cells transplantation can effectively increase the rate of skin flap regeneration and promote choke vasodilation.
