基于“十二皮部”理论的腕踝针对痔术后即时镇痛的临床研究

摘　　要：目的:基于“十二皮部”理论运用腕踝针对肛肠痔术后即时镇痛的临床疗效观察。方法:将60例混合痔术后患者随机分为针刺组、对照组各30例,两组患者均采用混合痔外剥内扎术,术后即时镇痛观察组给予脚踝针,选取患者双侧下1区(外踝最高点上三横指,靠跟腱内缘)、下6区(外踝最高点上三横指,靠跟腱外缘),以胶布固定留针6小时6h,对照组给予氨酚曲马多片1片口服,观察两组患者术后2h、4h、6h、8h的疼痛VAS评分,同时记录患者治疗有效情况;结果:两组治疗后6h、8h疼痛VAS评分比较,针刺组较对照组有较好的疗效(P<0.05),差异有统计学意义。术后两组急性尿潴溜情况比较,对照组发生率较针刺组高,差异有显著统计学意义(P<0.05)。术后6h两组5-HT情况比较,5-HT血清升高,镇痛效果明显,针刺组较对照组更高(P<0.05),差异有统计学意义。结论:腕踝针对肛肠病痔术后即时镇痛有较好的疗效,可减轻患者疼痛及恐惧感,降低了术后并发症的发生率。Objective:Based on the theory of“twelve meridional dermomeres”,the clinical efficacy of wrist-ankle acupuncture in immediate analgesia after anorectal hemorrhoidal surgery was observed.Methods:60patients with mixed hemorrhoids were randomly divided into observation group and control group,30cases in each group.Both groups underwent external peeling and internal ligation of mixed hemorrhoids,The postoperative immediate analgesia observation group was given to select the patient's bilateral lower 1area(three transverse fingers on the highest point of the lateral malleolus,against the inner edge of the Achilles tendon),lower 6areas(three transverse fingers at the highest point of the lateral malleolus,against the outer edge of the Achilles tendon),and fixed the needle with adhesive tape for 6hours.The control group was given 1tablet of aminophentramadol tablets orally,the pain VAS scores of 2h,4h,6h,and 8h after surgery were observed,and the effective treatment of the patients was recorded.Results:Compared with the VAS scores of pain at 2h,4h,6h and 8h after treatment,the acupuncture group had a better efficacy than the control group,and the difference was statistically significant(P<0.05).The incidence of acute urinary retention in the control group was significantly higher than that in the acupuncture group(P<0.05).Compared with the control group,the 5-HT serum of the acupuncture group increased,and the analgesic effect was obvious,and the difference was statistically significant(P<0.05).Conclusion:wrist-ankle acupuncture has better efficacy in immediate analgesia after hemorrhoidal surgery for anorectal disease,it can reduces patients'pain and fear,and reduces the incidence of postoperative complications.
