红参温肾助阳方对难治性心力衰竭阳虚水泛证患者血清心肌肌钙蛋白、骨桥蛋白水平的影响

摘　　要：目的:研究红参温肾助阳方对难治性心力衰竭阳虚水泛证患者血清心肌肌钙蛋白、骨桥蛋白水平的影响。方法:选取2020年3月~2022年3月我院收治的94例难治性心力衰竭阳虚水泛证患者作为研究对象,随机分为对照组(n=47)与观察组(n=47),对照组采用常规治疗方法(包括卧床休息、吸氧,给予醛固酮受体拮抗剂、正性肌力药、利尿剂等常规抗心力衰竭药物),观察组在对照组的基础上给予红参温肾助阳方治疗,比较两组临床疗效、中医症状积分、血清学指标、心功能及不良反应发生率。结果:观察组临床总有效率(95.74%)高于对照组(78.72%,P<0.05);治疗后,观察组各项中医症状积分均低于对照组(P<0.05);治疗后,两组患者心肌肌钙蛋白Ⅰ(cTnⅠ)、骨桥蛋白、N末端B型利钠肽前体(BT-proBNP)水平均降低,且观察组明显低于对照组(P<0.05);治疗后,两组患者左室射血分数(LVEF)、心排血量(CO)及每搏输出量(SV)水平均升高,且观察组明显高于对照组(P<0.05);两组不良反应发生率比较无明显差异(P>0.05)。结论:红参温肾助阳方治疗难治性心力衰竭阳虚水泛证临床疗效显著,可降低患者血清心肌肌钙蛋白、骨桥蛋白水平,改善心功能,且无明显不良反应发生,有较好的安全性。Objective To study the effects of Hongshen Wenshen Zhuyang decoction on levels of serum cardiac troponin and osteopontin in the treatment of refractory heart failure edema syndrome due to Yang deficiency.Methods 94 patients with refractory heart failure of edema syndrome due to Yang deficiency who were treated in the hospital between March 2020 and March 2022 were selected as the research subjects,and they were randomly divided into control group(n=47)and observation group(n=47).The control group was given conventional treatment methods(including bed rest,oxygen inhalation,conventional anti-heart failure drugs such as aldosterone receptor antagonists,inotropes and diuretics),while the observation group adopted Hongshen Wenshen Zhuyang decoction on the basis of the control group.The clinical efficacy,TCM symptoms scores,serological indicators,cardiac function and incidence rates of adverse reactions were compared between the two groups.Results The total clinical effective rate of 95.74%in observation group was higher than 78.72%in control group(P<0.05).After treatment,the scores of various TCM symptoms in observation group were lower compared with those in control group(P<0.05).The levels of cardiac troponin I(cTnI),osteopontin,and N-terminal B-type natriuretic peptide precursor(BT-proBNP)were all decreased in both groups after treatment,and the levels in observation group were significantly lower(P<0.05).The left ventricular ejection fraction(LVEF),cardiac output(CO)and stroke volume(SV)in the two groups were enhanced after treatment,and the above three indicators were significantly higher in observation group than control group(P<0.05).There was no significant difference in the occurrence rate of adverse events between the two groups(P>0.05).Conclusion Hongshen Wenshen Zhuyang decoction has a significant clinical efficacy in the treatment of refractory heart failure of edema syndrome due to Yang deficiency.It can reduce the levels of serum cardiac troponin and osteopontin,improve the cardiac function,and has a g
