特异性免疫疗法联合责任制护理对变应性鼻炎的疗效观察

摘　　要：目的探讨特异性免疫疗法(specific immunotherapy,SIT)联合责任制护理对变应性鼻炎(allergic rhinitis,AR)的疗效观察。方法选取2020年7月至2022年7月浙江大学医学院附属第二医院临平院区收治的AR患者110例,根据随机数字表法将患者分为对照组和研究组,每组各55例。对照组患者给予地氯雷他定联合常规护理,研究组患者给予SIT联合责任制护理;比较两组患者的每日用药评分、免疫功能、炎症因子、AR症状评分、生活质量、小气道功能指标、自我护理能力及护理满意度。结果干预后,两组患者的每日用药评分、白细胞介素(interleukin,IL)-2、IL-4、IL-5、γ干扰素(interferonγ,IFN-γ)、血管细胞黏附分子1(vascular cell adhesion molecule 1,VCAM-1)、免疫球蛋白E(immunoglobulin E,IgE)、AR症状评分、生活质量评分均显著低于本组干预前,最大呼气中期流量(maximal mid-expiratory flow,MMEF)、用力呼出50%肺活量的呼气流速(maximal expiratory flow at 50%of forced vital capacity,MEF50)、用力呼出25%肺活量的呼气流速(maximal expiratory flow at 25%of forced vital capacity,MEF25)、自我护理能力评分均显著高于本组干预前(P<0.05);研究组患者的每日用药评分、IL-2、IL-4、IL-5、IFN-γ、VCAM-1、IgE、AR症状评分、生活质量评分均显著低于对照组,MMEF、MEF50、MEF25、自我护理能力评分均显著高于对照组(P<0.05)。研究组患者的护理满意度显著高于对照组(χ^(2)=7.040,P=0.007)。结论SIT联合责任制护理可有效调节AR患者的免疫功能,抑制炎症反应,改善临床症状,有助于提高患者的健康知识水平、自护能力、生活质量和护理满意度。Objective To investigate the effect of specific immunotherapy(SIT)combined responsibility nursing on allergic rhinitis(AR).Methods A total of 110 AR patients admitted to Linping District,the Second Affiliated Hospital,Zhejiang University School of Medicine from July 2020 to July 2022 were selected and divided into control group and study group according to random number table method,with 55 cases in each group.The control group was given desloratadine combined with routine nursing care,while the study group was given SIT combined responsibility nursing care.Daily medication score,immune function,inflammatory factors,AR symptom score,quality of life,small airway function index,self-care ability and nursing satisfaction were compared between the two groups.Results After intervention,daily medication score,interleukin(IL)-2,IL-4,IL-5,interferon-γ(IFN-γ),vascular cell adhesion molecule 1(VCAM-1),immunoglobulin E(IgE),AR symptom score and quality of life score of the two groups were significantly lower than before intervention,and maximal mid-expiratory flow(MMEF),maximal expiratory flow at 50%of forced vital capacity(MEF50),maximal expiratory flow at 25%of forced vital capacity(MEF25)and the scores of self-care ability were significantly higher than before intervention(P<0.05).The daily medication score,IL-2,IL-4,IL-5,IFN-γ,VCAM-1,IgE,AR symptom score and quality of life score of patients in study group were significantly lower than those in control group,and MMEF,MEF50,MEF25 and scores of self-care ability were significantly higher than those in control group(P<0.05).The nursing satisfaction of study group was significantly higher than that of control group(χ^(2)=7.040,P=0.007).Conclusion SIT combined responsibility nursing can effectively regulate the immune function of AR patients,inhibit inflammatory response,improve clinical symptoms,and help improve patients’health knowledge,self-care ability,quality of life and nursing satisfaction.
