猪肺磷脂注射液联合无创正压通气对新生儿呼吸窘迫综合征的效果

摘　　要：目的探讨猪肺磷脂注射液联合无创正压通气(non-invasive positive pressure ventilation,NIPPV)治疗新生儿呼吸窘迫综合征(neonatal respiratory distress syndrome,NRDS)的效果。方法将新生儿科收治的80例NRDS患儿用随机数字表法分为联合组和对照组,每组40例。对照组予以NIPPV和常规方案治疗,联合组在对照组治疗的基础上予以猪肺磷脂注射液治疗。比较2组患儿治疗后的临床疗效、治疗情况、并发症发生率及治疗前后氧合指数。结果联合组的总有效率(92.50%)显著高于对照组(70.00%),P<0.05;联合组的撤机时间、氧疗时间和住院总时间均显著短于对照组(P<0.05);联合组的动脉血氧分压(partial pressure of blood oxygen,PaO_(2))/吸入氧浓度(fraction of inspired oxygen,FiO_(2))值、氧合指数(oxygenation index,OI)均显著高于对照组(P<0.05);呼吸指数(respiratory index,RI)、不良反应发生率显著低于对照组(P<0.05)。结论猪肺磷脂注射液联合NIPPV治疗NRDS的疗效显著,可有效改善患儿的氧合功能,降低并发症的发生率,安全性高。Objective To explore the clinical effects of Porcine Lung Phospholipid Injections combined with non-invasive positive pressure ventilation(NIPPV)in the treatment of children with neonatal respiratory distress syndrome(NRDS).Methods A total of 80 children with NRDS admitted to the neonatology department of the hospital were divided into combination group and control group according to random number table method,40 cases in each group.The control group was given NIPPV and routine treatment,while the combination group was additionally given Porcine Lung Phospholipid Injections.The clinical curative effect,treatment status,the incidence of complications after treatment,and the oxygenation index before and after treatment were compared between the 2 groups.Results After treatment,the total response rate of clinical effect in combination group was significantly higher than that in control group(92.50%vs.70.00%,P<0.05).The weaning time,oxygen therapy time and total hospitalization time in combination group were significantly shorter than those in control group(P<0.05).The partial pressure of blood oxygen(PaO_(2))/fraction of inspired oxygen(FiO_(2))and oxygenation index(OI)in combination group were significantly higher than those in control group(P<0.05),while the respiratory index(RI)and incidence of adverse reactions were significantly lower than those in control group(P<0.05).Conclusion The curative effect of Porcine Lung Phospholipid Injections combined with NIPPV is significant on NRDS,which can effectively improve oxygenation function and reduce the incidence of complications,with high safety.
