缙云地区女性人乳头瘤病毒感染情况及基因亚型分析

摘　　要：目的 分析人乳头瘤病毒(HPV)在缙云某医院就诊女性中的感染状况和基因亚型分布特征,为本地区HPV疫苗接种及科学防治提供理论依据。方法 选取2021年3月—2023年2月在缙云县人民医院就诊的女性11 956例,分析HPV的感染率、感染亚型、季节及各年龄段的分布特点。结果 在11 956例样本中,检出HPV感染者2 131例,感染率为17.82%。高危型感染2 185例,检出率为18.28%,高危型检出率前5位分别为HPV52(3.66%)、HPV58(2.73%)、HPV53(2.55%)、HPV16(1.64%)及HPV51(1.02%)。低危型感染502例,检出率为4.20%,低危型检出率前3位分别为HPV81型(1.31%)、HPV42型(1.08%)、HPV70(0.74%)。单一亚型感染者1 710例,感染率为14.30%,双重感染者309例,感染率为2.58%,三重及以上亚型感染112例,感染率为0.94%。HPV感染率存在季节性变化,不同年龄组HPV感染率近似“U”曲线,>65岁人群感染率最高,为40.88%,其次是56岁~65岁组和≤25岁组,感染率分别为37.10%和34.56%。不同年龄组HPV基因亚型分布有所差异,与其他年龄段相比,26岁~35岁组HPV各亚型分布处于较平均水平;而HPV6和HPV11亚型在<25岁组中比较突出;HPV52型亚型在36岁~45岁组中比较突出;HPV81型亚型在46岁~55岁组中比较突出;HPV31和HPV42型亚型在56岁~65岁组中比较突出;HPV16和HPV70亚型在>65岁组中比较突出。结论 缙云地区女性HPV感染以高危型和单一亚型感染为主,存在季节性变化,>65岁人群感染率最高,应重点关注,不同年龄组HPV基因亚型分布有所差异,应进行HPV疫苗精准接种和防治。Objective This paper aims to analyze the infection status and genotype distribution of female human papillomavirus(HPV)in Jinyun,so as to provide theoretical basis for the scientific prevention and treatment.Methods A total of 11956 patients who underwent HPV examination in Jinyun People’s Hospital from March 2021 to February 2023 were selected.The positive rate,genotype distribution,season and the infection characteristics of HPV subtypes in different age groups were analyzed.Results Among 11956 samples,2131 cases were positive for HPV(17.82%).Among them 2185 samples were high-risk subtypes(18.28%).The top five high-risk subtypes were HPV52(3.66%),HPV58(2.73%),HPV53(2.55%),HPV16(1.64%)and HPV51(1.02%).Meanwhile,502 samples were low-risk subtypes(4.20%).The top three low-risk subtypes were HPV81(1.31%),HPV42(1.08%)and HPV70(0.74%).1710 samples were single infection(14.30%),309 samples were double infection(2.58%),and 112 samples were the triple and multiple infections(0.94%).The infection rate of HPV was of obvious seasonality and similar to U curve in age groups.The infection rate in the group over 65 years old was the highest(40.88%),followed by the 56-65 years old group(37.10%)and the≤25 years old group(34.56%)respectively.The infection characteristics of HPV subtypes in different age groups were discrepancy.The distribution of HPV subtypes in the 26-35 age group was at average level,while the HPV6 and HPV11 subtypes were more prominent in the group of<25 years old;the HPV52 subtype was more prominent in the 36-45 years old group,the HPV81 subtype was more prominent in the 46-55 years old group,the HPV31 and HPV42 subtypes were more prominent in the 56-65 years old group,and the HPV16 and HPV 70 subtypes were more prominent in the>65 years old group compared with other age groups.Conclusion The high-risk subtypes and the single type infections of HPV were dominant in Jinyin area,which was of obvious seasonality.People>65 years old group have the highest infection rate and should be particularly concerned.T
