胫骨内侧高位截骨术后胫骨平台后倾角与膝关节功能评分的相关性

摘　　要：目的:观察膝关节骨性关节炎胫骨内侧高位截骨手术前后胫骨平台后倾角与膝关节功能评分的相关性。方法:回顾性分析自2021年1月至2022年1月,采用关节镜手术联合胫骨内侧高位截骨术治疗的60例膝关节骨性关节炎患者,术后1,3,12个月于侧位X线片测量胫骨平台后倾角,同时对患者进行膝关节功能评分。采用Pearson相关性分析术后胫骨平台后倾角与患者膝关节功能评分的相关性。结果:60例患者均获得12个月随访,随访期间未出现感染、截骨端不愈合、钢板断裂等并发症。术前胫骨平台后倾角为2.74°±0.41°,术后1个月为3.28°±0.82°,术后3个月为6.18°±1.12°,术后12个月为7.15°±2.08°。术前膝关节功能评分为(55.68±5.72)分,术后1个月为(78.98±6.32)分,术后3个月为(82.28±6.59)分,术后12个月为(88.11±7.21)分。Pearson相关性分析结果显示,术后胫骨平台后倾角与术后膝关节功能评分正相关(r=0.625,P<0.001)。结论:胫骨内侧高位截骨术后患者胫骨平台后倾角会变大,与术后患者膝关节功能恢复情况密切相关。Objective:To observe the correlation between the posterior inclination of tibial plateau and knee function score before and after high medial tibial osteotomy for knee osteoarthritis.Methods:A retrospective analysis was performed on 60 cases of knee osteoarthritis treated by arthroscopic surgery combined with medial high tibial osteotomy from January 2021 to January 2022.The posterior tilt of tibial plateau was measured by lateral radiographs 1,3 and 12 months after surgery,and knee function scores were performed.Pearson correlation was used to analyze the correlation between the posterior inclination of tibial plateau and the knee function score of patients after surgery.Results:All 60 cases were followed up for 12 months,during which there were no complications such as infection,nonunion of osteotomy end and plate fracture.The posterior inclination of tibial plateau was 2.74°±0.41°before surgery,3.28°±0.82°one month after surgery,6.18°±1.12°three months after surgery,and 7.15°±2.08°twelve months after surgery.The knee function score was(55.68±5.72)points before surgery,(78.98±6.32)points one month after surgery,(82.28±6.59)points three months after surgery,and(88.11±7.21)points twelve months after surgery.The result of Pearson correlation analysis showed that the posterior inclination of tibial plateau was positively correlated with the postoperative knee function score(r=0.625,P<0.001).Conclusion:The posterior inclination of tibial plateau decreases after high medial tibial osteotomy,which is closely related to the functional recovery of knee joint.
