摘 要:目的探究新生儿高胆红素血症中谷氨酰转肽酶(GGT)、肌酸激酶同工酶(CKMB)、促红细胞生成素(EPO)水平与总胆红素(TBil)相关性。方法选取2020年10月—2022年10月于诸暨市妇幼保健院产科分娩的75例高胆红素血症新生儿,且符合足月儿黄疸干预标准作为试验组研究对象。按总胆红素水平分为轻度升高组29例,中度升高组19例,重度升高组27例,按疾病类型分为病理性组29例,生理性组46例,另外同期选取35例无黄疸的正常足月新生儿作为对照组研究对象。分析GGT、CKMB、EPO、TBil水平在新生儿高胆红素血症中的表达及作用。采用Pearson相关性分析软件对新生儿高胆红素血症中GGT、CKMB、EPO水平与总胆红素相关性进行分析。结果试验组GGT、CKMB、EPO、TBil水平高于对照组(P<0.05)。病理性组GGT、CKMB、EPO、TBil水平高于生理性组(P<0.05)。重度升高组GGT、CKMB、EPO、TBil水平高于中度升高组(P<0.05)。GGT、CKMB、EPO、TBil水平与高胆红素血症患者病情均成正相关(P=0.001)。经logistic线性回归分析显示GGT、CKMB、EPO是造成高胆红素血症新生儿TBil水平上升的影响因素。结论GGT、CKMB、EPO在新生儿高胆红素血症中异常高表达,且与疾病类型以及TBil水平程度相关,监测GGT、CKMB、EPO水平可利于诊断新生儿高胆红素血症。Objective This paper aims to explore the correlation between glutamyl transpeptidase(GGT),creatine kinase isoenzyme(CKMB),erythropoietin(EPO)levels and total bilirubin(TBil)in neonatal hyperbilirubinemia.Methods A total of 75 neonates with hyperbilirubinemia who were delivered in the Department of Obstetrics in Zhuji Maternal and Child Health Care Hospital from October 2020 to October 2022 and met the criteria for intervention of jaundice in term infants were selected as the study subjects in the test group.According to the total bilirubin level,29 cases were classified as mild increased group,19 cases as moderate increased group,27 cases as severe increased group,29 cases as pathological group and 46 cases as physi⁃ological group according to the types of disease.In addition,35 normal term newborns without jaundice were selected as the control group during the same period of study.The expression and roles of GGT,CKMB,EPO and TBIL in neonatal hyperbiliru⁃binemia were analyzed.Pearson's correlation analysis software was used to analyze the correlation between GGT,CKMB and EPO levels and total bilirubin in neonatal hyperbilirubinemia.Results GGT,CKMB,EPO,TBid in test group were higher than that in the control group(P<0.05).GGT,CKMB,EPO,TBid were higher than that in the physiological group(P<0.05).In the severe increase group,GGT,CKMB,EPO,TBid were higher than that in the moderately elevated group(P<0.05).GGT,CKMB,EPO,and TBil were all positively associated with the condition of patients with hyperbilirubinemia(P=0.001).Logistic linear regression analysis showed that GGT,CKMB and EPO were influencing factors for the increase of TBil in neonates with hyperbilirubinemia.Conclusion GGT,CKMB,and EPO are abnormally highly expressed in neonatal hyper⁃bilirubinemia,and are related to the type of disease and the level of TBil.Monitoring the levels of GGT,CKMB,and EPO can help diagnose neonatal hyperbilirubinemia.
