摘　　要：糖尿病足溃疡(DFU)是糖尿病的严重并发症之一,发病率和病死率均较高,严重危害患者健康。细胞外基质(ECM)组成成分具有止血、抑炎、促增殖、增加弹力等生物学作用,在创面修复过程中发挥强大的支撑和调控效应,是DFU治疗的关键。基质金属蛋白酶(MMP)作为ECM的衍生物质,种类繁多、分工复杂,主要通过降解和重塑ECM调控创面愈合进程,其中MMP-1、MMP-2和MMP-9在DFU中大量表达,导致ECM被过度降解,引发DFU愈合困难,而MMP-8可通过重塑ECM促进DFU愈合。因此,合理调控ECM/MMP平衡可以为DFU的治疗提供新思路。Diabetic foot ulcer(DFU)is one of the serious complications of diabetes,with high morbidity and mortality,which seriously endangers the health of the patients.The extracellular matrix(ECM)components have such biological effect as stopping bleeding,inhibiting inflammation,promoting proliferation,increasing elasticity,which play a strong support and regulatory effect in the process of wound repair,and is regarded as the key of DFU treatment.Matrix metalloproteinase(MMP),as derivatives of ECM,have many kinds and complicated functions,which regulates the process of wound healing mainly by degrading and remodeling ECM.MMP-1,MMP-2 and MMP-9 are massively expressed in DFU,resulting in excessive degradation of ECM and difficulties in DFU healing,while MMP-8 can reshape ECM to promote DFU healing.Therefore,reasonable regulation of ECM/MMP balance in DFU may provide new ideas for the treatment.
