摘  要:目的分析低强度超声辐照联合静脉注射造影剂微泡对急性心肌梗死(AMI)大鼠微循环的改善作用,并探究其内在机制。方法将造模成功的45只大鼠按随机数字表法分为AMI组、超声组、超声辐照联合SonoVue造影剂微泡组(联合组),每组15只。选取15只健康雄性SD大鼠作为正常对照组。其中,正常对照组、AMI组按照同样方式饲养,不做处理;超声组大鼠舌下静脉滴注20 ml 0.9%氯化钠溶液联合超声辐照心脏;联合组给予SonoVue微泡联合超声辐照心脏。比较各组大鼠的超声检测指标[左心室舒张末期内径(LVEDD)、室间隔厚度(IVS)、左心室收缩末期内径(LVESD)、左心室射血分数(LVEF)]变化,心肌染色结果、心肌氧化应激及炎症检测结果[丙二醛(MDA)、超氧化物歧化酶(SOD)、白细胞介素(IL)-1β、IL-6]以及心肌组织的Western Blot检测结果[SOD2、核因子κB(NF-κB)、核苷酸结合寡聚化结构域样受体蛋白3(NLRP3)和IL-1前体(pro-IL-1)]。结果实验干预14 d后,正常对照组大鼠全部存活,AMI组大鼠存活10只,超声组大鼠存活11只,联合组大鼠存活14只。AMI组和超声组的LVEDD高于正常对照组[(82.0±3.2)mm、(82.7±2.2)mm比(79.3±2.8)mm](P<0.05)。联合组的LVESD低于AMI组和超声组[(49.1±2.4)mm比(55.0±4.8)mm、(53.2±4.5)mm],LVEF高于AMI组和超声组[(74.3±3.2)%比(66.8±5.4)%、(70.0±5.8)%](P<0.05)。联合组的毛细血管密度较AMI组和超声组明显增大,胶原纤维数明显减少,存活的心肌也最多。联合组的MDA、IL-1β、IL-6水平低于AMI组和超声组[(4525±269)mmol/g比(9306±1359)mmol/g、(9857±647)mmol/g,(160.7±10.8)ng/L比(280.4±18.6)ng/L、(265.4±23.4)ng/L,(95.3±9.4)ng/L比(136.0±10.8)ng/L、(131.8±10.9)ng/L],SOD水平高于AMI组和超声组[(89.6±5.1)U/mg比(54.5±3.0)U/mg、(60.1±4.1)U/mg](P<0.01)。联合组的SOD2蛋白表达水平高于AMI组、超声组(1.87±0.17比1.33±0.19、1.11±0.18)(P<0.01),联合组的NF-κB、NLRP3和pro-IL-1蛋�Objective To analyze the effect of low-intensity ultrasound irradiation combined with intravenous injection of contrast microbubbles on the improvement of microcirculation in acute myocardial infarction(AMI)in rats,and to investigate the intrinsic mechanism.Methods Forty-five rats with successful modeling were divided into an AMI group,an ultrasound group,an ultrasound irradiation combined with SonoVue contrast agent microbubble group(combined group)according to random number table method,with 15 rats in each group.15 healthy male SD rats were selected as a normal control group.Among them,the normal control group and AMI group were fed in the same way without treatment;in the ultrasonic group,20 ml 0.9%sodium chloride solution was injected into the sublingual vein of rats and the heart was irradiated by ultrasound;the combined group was given SonoVue microbubbles combined with ultrasound to irradiate the heart.The changes of ultrasonic detection indexes[left ventricular end-diastolic diameter(LVEDD),interventricular septum(IVS),left ventricular end-systolic diameter(LVESD)and left ventricular ejection fraction(LVEF)]of rats in each group,as well as myocardial staining results,myocardial oxidative stress and inflammation detection results[malonic dialdehyde(MDA),superoxide dismutase(SOD),interleukin(IL)-1β,IL-6],Western Blot detection results of myocardial tissue[SOD2,nuclear factor-κB(NF-κB),nucleotide-binding oligomeric domain-like receptor protein 3(NLRP3)and pro-IL-1].Results After 14 d of experimental intervention,all rats in the normal control group survived,10 rats in the AMI group survived,11 rats in the ultrasound group survived,and 14 rats in the combined group survived.The LVEDD of the AMI group and ultrasound group was higher than that of the normal control group[(82.0±3.2)mm,(82.7±2.2)mm vs(79.3±2.8)mm](P<0.05).LVESD in the combined group was lower than that in the AMI group and ultrasound group[(49.1±2.4)mm vs(55.0±4.8)mm,(53.2±4.5)mm],and LVEF was higher than that in the AMI group and ultra
