肌骨超声引导下隐神经阻滞治疗膝关节骨性关节炎的效果分析

摘　　要：目的:分析肌骨超声引导下隐神经阻滞治疗膝关节骨性关节炎的临床效果。方法:抽取2020年7月至2022年7月郑州人民医院收治的膝关节骨性关节炎患者60例,按随机数字表法分为对照组与观察组,每组30例。对照组给予关节内注射玻璃酸钠治疗,观察组在对照组治疗基础上给予肌骨超声引导下隐神经阻滞。比较两组治疗效果、疼痛视觉模拟量表(VAS)评分、Lysholm膝关节康复量表评分、西安大略和麦克马斯特大学关节炎指数(WOMAC)评分、白细胞介素-1(IL-1)水平、白细胞介素-6(IL-6)水平。结果:观察组总有效率(96.67%,29/30)高于对照组(73.33%,22/30),P<0.05。治疗后1、3个月,观察组VAS评分、WOMAC评分低于对照组(t=12.37、22.26、6.76、9.36,P均<0.05),Lysholm评分高于对照组(t=4.72、4.43,P均<0.05),IL-1、IL-6水平低于对照组(t=6.07、5.45、10.37、17.66,P均<0.05)。结论:肌骨超声引导下隐神经阻滞治疗膝关节骨性关节炎效果确切,可有效减轻炎性反应,改善膝关节功能,减轻关节病损程度。Objective To analyze the effect of saphenous nerve block guided by musculoskeletal ultrasound in the treatment of knee osteoarthritis.Methods A total of 60 patients with knee osteoarthritis admitted to People’s Hospital of Zhengzhou from July 2020 to July 2022 were selected,and the patients were divided into control group and observation group by random number table method,with 30 cases in each group.The control group was treated with intra-articular injection of sodium hyaluronate,and the observation group was given saphenous nerve block guided by musculoskeletal ultrasound based on the treatment of the control group.The treatment effects,scores of visual analogue scale(VAS),Lysholm knee rehabilitation scale and Western Ontario and McMaster Universities arthritis index(WOMAC),and levels of interleukin-1(IL-1)and interleukin-6(IL-6)were compared between the two groups.Results The total effective rate in the observation group(96.67%,29/30)was higher than that in the control group(73.33%,22/30),P<0.05.After 1 and 3 months of treatment,the observation group had lower scores of VAS and WOMAC(t=12.37,22.26,6.76,9.36;all P<0.05),higher Lysholm score(t=4.72,4.43;all P<0.05),and the lower levels of IL-1 and IL-6(t=6.07,5.45,10.37,17.66;all P<0.05),compared with the control group.Conclusions Saphenous nerve block guided by musculoskeletal ultrasound is effective in the treatment of knee osteoarthritis,which can effectively reduce the inflammatory response,improve knee joint function,and reduce the degree of joint damage.
