彩色多普勒超声诊断下肢骨折术后深静脉血栓的价值

摘　　要：目的:探讨彩色多普勒超声诊断下肢骨折术后深静脉血栓的价值。方法:抽取2020年1月至2022年12月濮阳市人民医院收治的疑似下肢骨折术后深静脉血栓患者73例,均接受彩色多普勒超声诊断,以静脉造影结果为"金标准",分析彩色多普勒超声对下肢骨折术后深静脉血栓的诊断效能,比较下肢骨折术后深静脉血栓阳性者与阴性者彩色多普勒超声测量的血管内径、血流速度。结果:静脉造影结果显示,73例患者中27例有下肢骨折术后深静脉血栓,彩色多普勒超声诊断下肢骨折术后深静脉血栓的敏感性为92.59%(25/27),特异性为91.30%(42/46),准确性为91.78%(67/73)。彩色多普勒超声结果显示,下肢骨折术后深静脉血栓阳性者股浅静脉、双侧股总静脉、内侧腓肠静脉、胫周围静脉近端血管内径小于阴性者(P<0.05),腘静脉血管内径两者比较差异未见统计学意义(P>0.05);下肢骨折术后深静脉血栓阳性者下肢静脉收缩期峰值流速、舒张末期流速小于阴性者(P<0.05),阻力指数大于阴性者(P<0.05)。结论:彩色多普勒超声能有效显示下肢骨折术后患者下肢深静脉血管内径和血流速度,在下肢骨折术后深静脉血栓诊断中具有较高的敏感性、特异性和准确性。Objective To investigate the value of color Doppler ultrasonography in diagnosing deep vein thrombosis after lower limb fracture operation.Methods Seventy-three patients with suspected deep vein thrombosis after lower limb fracture operation admitted to Puyang People’s Hospital from January 2020 to December 2022 were selected,and them all received color Doppler ultrasound examination for diagnosis.Taking the results of venography as the gold standard,the efficiency of color Doppler ultrasound in the diagnosis of deep vein thrombosis after lower extremity fracture operation was analyzed.The blood vessel diameter and blood flow velocity measured by color Doppler ultrasonography were compared between patients who were positive and patients who were negative for deep vein thrombosis after lower extremity fracture operation.Results The results of venography showed that 27 of 73 selected patients had deep vein thrombosis after lower limb fracture operation.Color Doppler ultrasonography had the sensitivity of 92.59%(25/27),the specificity of 91.30%(42/46),and the accuracy of 91.78%(67/73)in diagnosing deep vein thrombosis after lower extremity fracture operation.The results of color Doppler ultrasonography showed that the diameter of the superficial femoral vein,bilateral common femoral vein,medial sural vein,and peritibial vein proximal vessel in patients with positive deep venous thrombosis after lower extremity fracture operation were smaller than those in negative patients(P<0.05).There was no significant difference in the diameter of the popliteal vein between the two types of patients(P>0.05).The peak systolic velocity and end-diastolic velocity of lower extremity venous in patients with positive deep venous thrombosis after lower extremity fracture operation were lower than those in negative patients(P<0.05),and the resistance index was higher than that in negative patients(P<0.05).Conclusions Color Doppler ultrasonography can effectively display the diameter and blood flow velocity of deep vein vessels in pati
