摘  要:目的探究二氧化氯溶液对呼吸重症监护病房(RICU)环境多药耐药菌(MDROs)的清洁消毒效果。方法2019年2-3月RICU环境使用500 mg/L含氯消毒剂进行清洁消毒,设为含氯消毒剂组;2019年4-12月RICU环境使用二氧化氯消毒液制取设备制备的80~100 mg/L二氧化氯溶液进行清洁消毒,并对MDROs患者周围环境增加一次消毒,设为二氧化氯组;对两组环境物体表面和地面采样进行细菌检测分析,研究二氧化氯溶液对RICU环境多药耐药菌的清洁消毒效果。结果二氧化氯组患者周围高频接触物体表面和地面细菌检出率低于含氯消毒剂组(P<0.001);医护办公区高频接触物体表面细菌检出率也有不同程度下降,但差异无统计学意义(P=0.210);两组环境中检出的MDROs以耐碳青霉烯鲍氏不动杆菌(CRAB)为主,与患者检出的MDROs基本一致。结论二氧化氯消毒液制取设备操作简便,能提高保洁人员使用的依从性,制备的80~100 mg/L二氧化氯溶液对环境表面具有良好的消毒效果;强化清洁消毒可降低MDROs患者周围环境细菌检出率。OBJECTIVE To investigate the effect of chlorine dioxide solution on cleaning and disinfection of multidrug-resistance organism(MDROs)in the environment of Respiratory Intensive Care Unit(RICU).METHODS From Feb.to Mar.2019,the RICU environment was cleaned and disinfected with 500mg/L chlorine containing disinfectant,and was assigned to the chlorine containing disinfectant group.From Apr.to Dec.2019,RICU environment was cleaned and disinfected using 80~100mg/L chlorine dioxide solution prepared by the chlorine dioxide disinfectant preparation equipment for 0 cleaning and disinfection,and one additional disinfection was added to the environment around the patients with MDROs,which was set up as the Chlorine dioxide group.The bacteria on the surface of the objects and ground samples of environment of two groups were detected and analyzed to study the cleaning and disinfection effect of chlorine dioxide solution on multidrug resistance in RICU environment.RESULTS The detection rate of bacteria on the surface and ground surrounding high-frequency contact objects in the chlorine dioxide group was lower than that in the chlorine containing disinfectant group(P<0.001),with a statistically significant difference.The detection rate of bacteria on the surface of high-frequency contact objects in cant(P=0.210).The MDROs detected in the environments of the two groups were dominated by carbapenem-resistant Acinetobacter baumannii(CRAB),which was basically consistent with the MDROs detected in patients.CONCLUSION The equipment for preparing chlorine dioxide disinfectant was easy to operate,which could improve the compliance of cleaning staff to use it.The prepared 80-100mg/L chlorine dioxide solution had a good disinfection effect on environmental surfaces.Enhanced cleaning and disinfection could reduce the bacterial detection rate of environment around patients with MDROs.
