Ilizarov骨转运与Masquelet技术治疗胫骨感染性骨不愈合的前瞻性比较

摘　　要：目的:比较Ilizarov骨转运和Masquelet技术在治疗胫骨感染性骨不愈合方面的临床效果,为今后的临床工作提供切实可靠的依据。方法:回顾性分析2018年1月—2021年1月南部战区总医院收治的25例胫骨感染性骨不愈合病例的临床资料。根据治疗方式的不同,分为组1(13例)和组2(12例)。组1采用Ilizarov骨转运技术治疗,组2采用Masquelet技术治疗。比较两组患者一般资料、固定时间、外固定器指数、骨愈合结果、功能结果。结果:组1固定时间较组2固定时间短,差异有统计学意义(P<0.05)。组1外固定器指数[(2.48±0.56)月/cm]较组2外固定器指数[(4.71±1.71)月/cm]小,差异有统计学意义(P<0.05)。组1骨愈合优秀9例,良好3例,一般1例;组2优秀3例,良好3例,一般6例,两组患者骨愈合结果比较,差异有统计学意义(P<0.05)。组1功能结果优秀8例,良好4例,一般1例;组2优秀2例,良好6例,一般1例,两组患者功能结果比较,差异有统计学意义(P<0.05)。结论:对于骨丢失<6 cm的感染性胫骨不愈合,Ilizarov骨转移技术相比Masqulet技术在骨愈合方面与功能恢复方面更具有优势。Objective:To compare the clinical outcomes of Ilizarov bone transfer and Masquelet technique in the treatment of infected bone nonunion in the tibia to provide a practical and reliable basis for future clinical work.Methods:The clinical data of 25 cases of infected bone nonunion in tibia admitted to the hospital from January 2018 to January 2021 were retrospectively analyzed.According to the different treatment modalities,they were divided into group 1 and group 2.Group 1 was treated with Ilizarov bone transfer technique and group 2 was treated with Masquelet technique.General data,fixation time,external fixator index,bone healing results,and functional results were compared between the two groups.Results:Group 1 had a shorter fixation time than group 2,and the difference was statistically significant(P<0.05).The external fixator index was smaller in group 1[(2.48±0.56)months/cm]than in group 2[(4.71±1.71)months/cm],and the difference was statistically significant(P<0.05).In group 1,there were 9 cases of excellent bone healing,3 cases of good,and 1 case of average,in group 2,there were 3 cases of excellent,3 cases of good,and 6 cases of average,and the difference in the results of bone healing between the two groups of patients was statistically significant(P<0.05).In group 1,there were 8 cases with excellent,4 cases with good,and 1 case with average functional results,in group 2,there were 2 cases with excellent,6 cases with good,and 1 case with average functional results,and the difference in functional results between the two groups of patients was statistically significant(P<0.05).Conclusion:For infected tibial nonunions with bone loss of less than 6 cm,the Ilizarov bone transfer technique is superior to the Masqulet technique in terms of bone healing with functional recovery.
