摘  要:目的:检测儿童腺样体肥大患者血清中肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白三烯D4(LTD4)的表达水平,探讨其与腺样体是否肥大及肥大程度、体质量指数(BMI)之间的关系。方法:选取2019年7月—2020年8月在荆州市中心医院耳鼻喉科就诊的78例单纯腺样体肥大患儿作为实验组,同期筛选82例体检健康的儿童作为对照组。测定两组儿童身高、体重,计算儿童BMI,采用腺样体与鼻咽腔比率(A/N)来评估儿童腺样体的肥大程度。检测两组儿童外周血清中TNF-α、IL-6、LTD4浓度水平,分析其与腺样体肥大程度、BMI之间的关系。结果:实验组TNF-α、IL-6、LTD4表达水平及BMI明显高于对照组,年龄低于对照组,差异有统计学意义(t=11.010、2.217、12.500、2.422、1.792,P<0.05)。腺样体肥大程度与TNF-α、IL-6、LTD4表达水平变化趋势均为轻度<中度<重度,差异有统计学意义(F=72.172、10.030、22.585,P<0.05)。结论:腺样体肥大患儿血清中TNF-α、IL-6、LTD4表达水平升高,与腺样体肥大程度有相关性,与BMI无明显相关性,单因素logistic回归分析表明儿童腺样体是否肥大与性别无关,与年龄、BMI、血清中TNF-α、IL-6、LTD4表达水平均有关,推测其可能参与了腺样体肥大增生过程。Objective:To detect the expression levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and leukotriene D4(LTD4)in the serum of children with adenoid hypertrophy,and to investigate the relationship between them and whether and to what extent the adenoids are hypertrophic,and the body mass index(BMI).Methods:78 children with simple adenoid hypertrophy who attended the hospital from July 2019 to August 2020 were selected as the experimental group,and 82 children with healthy physical examination were screened as the control group during the same period.The height and weight of the children in both groups were measured,the BMI of the children was calculated,and the adenoids to nasopharynx ratio(A/N)was used to assess the degree of adenoid hypertrophy in children.The levels of TNF-α,IL-6 and LTD4 in the peripheral serum of the children in both groups were measured,and the relationship between them and the degree of adenoid hypertrophy and BMI was analyzed.Results:The expression levels of TNF-α,IL-6,LTD4 and BMI in the experimental group were significantly higher than those in the control group,and the age was lower than that in the control group,with statistically significant differences(t=11.010,2.217,12.500,2.422,1.792,P<0.05).The trends in the degree of adenoid hypertrophy and the levels of TNF-α,IL-6,and LTD4 expression were mild<moderate<severe,with statistically significant differences(F=72.172,10.030,22.585,P<0.05).Conclusion:Elevated serum expression levels of TNF-α,IL-6,and LTD4 in patients with adenoid hypertrophy were correlated with the degree of adenoid hypertrophy and not significantly correlated with BMI.Univariate logistic regression analysis showed that the hypertrophy of adenoids in children was not related to gender,but to age,BMI,and serum expression levels of TNF-α,IL-6,and LTD4,which were presumed to be involved in the hyperplasia of adenoids.
